摘　　要：目的:探讨呼吸康复治疗技术对ICU困难脱机患者脱机成功率的影响。方法:将2018年12月至2020年6月期间佛山市中医院重症医学科收治的机械通气反复脱机困难的患者分为对照组(n=49)和治疗组(n=48)。对照组接受常规治疗,治疗组在对照组治疗基础上加用呼吸康复训练。比较两组治疗前后的氧合指数、机械通气时间、脱机成功率、ICU住院时间及28 d死亡率、机械通气相关并发症发生情况。结果:治疗组的机械通气时间、ICU住院时间和28 d死亡率明显下降,脱机成功率、氧合指数高于对照组,并发症发生率低于对照组,差异均具有统计学意义(P<0.05)。结论:呼吸康复治疗技术可提高重症医学科患者脱机成功率,缩短机械通气时间和ICU住院时间,改善患者的呼吸功能和氧合指数,减少并发症的发生率。Objective:To explore the effect of respiratory rehabilitation therapy on successful rate of weaning in ICU patients with difficult weaning.Methods:Patients with difficult weaning in ICU were included and divided into the control group(n=49)and treatment group(n=48)from Dec 2018 to Jun 2020.Patients in control group were given conventional care.Patients in treatment group were received respiratory rehabilitation therapy based on the control group.The improvement of the oxygenation index,ventilation duration,ICU stay,28⁃day mortality rate and the occurrence rate of related complications were compared between the two groups.Results:The characteristics of two groups were similar.Compared with control group,the ventilation duration,ICU stay and 28⁃day mortality rate were significantly decreased,and weaning successful rate and oxygenation index were much higher in the treatment group with lower complication rates,all P<0.05.Conclusion:Respiratory rehabilitation therapy can increase the successful rate of weaning,shorten the ventilation duration and ICU stay,improve the breathing function and oxygen index,as well as decrease related complications among ICU Patients with difficult weaning.
