手法整复结合铝板功能位固定治疗稳定型第五掌骨头下骨折60例

摘　　要：目的:观察手法整复结合铝板功能位固定治疗稳定型第五掌骨头下骨折的有效性和安全性。方法:选取稳定型第五掌骨头下骨折患者60例,均采用手法整复结合铝板功能位固定治疗。结果:60例患者均在4~6周愈合,末次随访时参照中医骨伤科病证诊断疗效标准评价,其中优42例,良16例,差2例,优良率为96.6%。60例患者均获得随访,随访时间2~5个月,平均4.1个月;所有患者均无手外观畸形、手指屈伸障碍、创伤性关节炎、骨化性肌炎等并发症。疼痛感均消失,可自主活动。手部皮肤均无压疮、张力性水疱等。结论:对于稳定型第五掌骨头下骨折,采取手法整复结合铝板功能位固定治疗,通常能取得很好的临床疗效,值得在临床推广应用。Objective:To observe the efficacy and safety of manual reduction combined with functional fixation of aluminum plate on the treatment of stable sub-cephalic fractures of the fifth metacarpal bone.Methods:60 patients with stable sub-cephalic fractures of the fifth metacarpal bone were treated by manual reduction combined with functional position fixation of aluminum plate.Results:All the 60 patients healed in 3 to 4 weeks, and the efficacy was evaluated according to the diagnostic criteria of TCM orthopedics and traumatology at the last follow-up, among which 42 cases were excellent, 16 cases were good, and 2 cases were poor.The excellent and good rate was 96.6%.All the 60 patients were followed up for 2 to 5 months, with an average of 4.1 months.None of the patients had hand deformity, finger flexion and extension disorder, traumatic arthritis and other complications.No pressure sores or tension blisters were found on the hand skin.Conclusion:For stable sub-cephalic fractures of the fifth metacarpal bone, manual reduction combined with functional fixation of aluminum plate can usually achieve good clinical efficacy, which is worthy of clinical application and promotion.
