应用微型外固定支架治疗19例锤状指的临床观察

摘　　要：目的:采用伊里扎洛夫微型外固定支架(Ilizarov Mini External Fixtor, IMEF)治疗锤状指,探讨其疗效和安全性。方法:回顾性分析2020年4月至2021年4月,应用IMEF技术治疗19例锤状指患者。其中男11例,女8例;年龄15~70岁,平均41岁。术后随访观察、记录患指功能及外形,并对疗效、安全性进行综合评价。结果:本组19例患者均获随访,随访时间3~15个月,中位数6个月。术后4~6周拆除外固定支架。疗效及安全性评定采用Crawford标准评定,优14例,良4例,可1例。根据指间关节主动活动度(Total Active Motion, TAM)标准评定,优13例,良5例,可1例。两类评分优良率均为94.7%。术中无血管神经损伤,术后出现1例钉道感染,无其他并发症。结论:应用IMEF技术治疗锤状指临床验证疗效优良,并发症少,值得推广应用。Objective:To report a new surgical technique of Ilizarov mini external fixator(IMEF) on the treatment of mallet finger, and to discuss its efficacy and safety.Methods:19 patients with mallet finger treated with Ilizarov mini external fixator from April 2020 to April 2021 were retrospectively analyzed, including 11 males and 8 females.The age ranged from 15 to 70 years old with 41 years old on average.The function and shape of the affected finger were observed and recorded, and the efficacy and safety were evaluated comprehensively.Results:All the 19 patients in this group were followed up for 3 to 15 months and at a medium of 6 months post-operation.All patients removed fixation 4 to 6 weeks after operation.Efficacy and safety were assessed by Crawford standard, 14 cases were excellent, 4 cases were good, and 1 case was fair.According to TAM standard, 13 cases were excellent, and 5 cases were good and 1 case was fair.The excellent and good rates of two classes of grade were 94.7%.There was no vascular and nerve injury during the operation.There was 1 case of nail opening infection after the operation, and no other complications.Conclusion:The application of IMEF technique on the treatment of mallet finger is proved to be effective and less complications, which is worthy of promotion.
