摘　　要：目的探讨老年股骨头缺血性坏死患者置换术后衰弱水平的影响因素。方法回顾性分析2019年4月-2020年4月在我院进行人工髋关节置换术治疗的86例老年股骨头缺血性坏死患者的临床资料,均于术后24h,采用衰弱筛查量表(FRAIL)评估患者的衰弱水平,而后设计一般资料调查问卷记录患者的相关资料,分析老年股骨头缺血性坏死患者置换术后衰弱水平的影响因素。结果 86例老年股骨头缺血性坏死患者置换术后衰弱率为20.93%;术后衰弱患者无配偶率高于未衰弱患者,卧床时间长于未衰弱患者,心理弹性量表评分低于未衰弱患者,差异有统计学意义(P<0.05);经Logistic回归分析结果显示,无配偶、卧床时间长、心理弹性量表评分低可能是老年股骨头缺血性坏死置换术后衰弱的影响因素(P<0.05)。结论无配偶、卧床时间长、心理弹性量表评分低可能是老年股骨头缺血性坏死置换术后衰弱的影响因素。Objective To investigate the influencing factors of weakness level in elderly patients with avascular necrosis of femoral head after arthroplasty.Methods The clinical data of 86 elderly patients with avascular necrosis of femoral head who underwent artificial hip arthroplasty in our hospital from Apr. 2019 to Apr. 2020 were retrospectively analyzed,24 hours after the operation,frailty screening scale(FRAIL) was used to evaluate the weakness level of patients,then a general information questionnaire was designed to record the relevant data of the patients,the influencing factors of weakness level in elderly patients with avascular necrosis of femoral head after arthroplasty were analyzed.Results Of 86 cases of elderly patients with avascular necrosis of femoral head after arthroplasty,the weakening rate was 20.93%;The rate of no spouse was higher, and the time in bed was longer than those without postoperative weakness,the score of psychological resilience scale was lower in patients with postoperative weakness than those without postoperative weakness, there was statistical significance in the differences(P <0.05);The results of Logistic regression analysis showed that no spouse,long time in bed,low score of psychological resilience scale might be the influencing factors of weakness in elderly patients with avascular necrosis of femoral head after arthroplasty(OR>1,P<0.05).Conclusions No spouse, long time in bed, low score of psychological resilience scale may be the influencing factors of weakness in elderly patients with avascular necrosis of femoral head after arthroplasty.
