网状脱细胞异体真皮联合自体刃厚皮片移植在体表创面愈合中的临床应用

摘　　要：目的探讨网状脱细胞异体真皮与自体刃厚皮片复合移植在体表创面愈合中的临床疗效。方法回顾性分析2019年9月至2020年12月郑州大学第一附属医院整形外科收治的45例体表肿物切除后创面植皮患者的临床资料,按照植皮方法分为网状脱细胞异体真皮联合自体刃厚皮片复合移植组(A组,n=20)和单纯自体中厚皮片移植组(B组,n=25)。比较两组患者拆包时间、住院时长、皮片成活率、术后有无并发症、二次手术率、术后植皮区瘢痕温哥华瘢痕评估量表(VSS)评分以及患者满意度得分。结果A组皮片存活率、术后有无并发症以及二次手术率与B组比较,差异无统计学意义(P>0.05)。A组拆包时间、住院时长和术后植皮区瘢痕VSS评分低于B组,患者满意度评分高于B组(P<0.05)。结论网状脱细胞异体真皮与自体刃厚皮片复合移植在保证皮片存活率的同时,缩短了住院时长,具有术后瘢痕较轻,患者满意度高等优点,相较于单纯的自体中厚皮片移植,具有较好的临床疗效。Objective To explore the clinical effect of reticular acellular allogeneic dermis with razor autologous skin graft on superficial wound healing.Methods The clinical data of 45 patients who received skin grafting after excision of superficial tumor in the Plastic Surgery Department of the First Affiliated Hospital of Zhengzhou University from September 2019 to December 2020 were analyzed retrospectively.According to the method of skin graft,the patients were divided into reticular acellular dermis combined with razor autologous skin graft group(group A,n=20)and simple autologous medium thickness skin graft group(group B,n=25).The unpacking time,hospitalization duration,skin graft survival rate,postoperative complications,second operation rate,postoperative Vancouver scar scale(VSS)score and satisfaction score between the two groups were compared.Results There were no stastical differences in skin graft survival rate,postoperative complications and second operation rate between group A and group B(P>0.05).The unpacking time,hospitalization duration and VSS score in group A were lower than those in group B,and the score of satisfaction in group A was higher than that in group B(P<0.05).Conclusion The compound transplantation of reticular acellular allogeneic dermis with razor autologous skin graft can not only guarantee the survival rate of skin grafts,but also shorten the hospitalization time.It has the advantages of light postoperative scar and high patient satisfaction.Compared with simple autologous medium thickness skin graft,it has better clinical efficacy.
