摘　　要：目的通过对部分低病毒载量人群的耐药分析,了解HIV患者中低病毒载量人群的耐药状况。方法选择2019年6月—2020年6月病毒载量低于1000 copy/ml正在治疗的患者的血浆87份,对其一般流行病学调查,完成HIV病毒载量检测、用in-house的方法完成HIV-1基因型耐药检测,通过专业软件对序列拼接、分析比对,完成耐药序列比对和基因亚型分析。结果87份低病毒载量人群中成功获得14份序列,其中4例产生不同程度的耐药,耐药率为4.6%。通过对亚型的分析显示,新疆仍然以CRF07-BC亚型为主要亚型,但已呈现复杂性,主要亚型依次为CRF07-BC亚型(12例,85.7%)、B亚型(1例,7.1%)、CRF01-AE亚型(1例,7.1%)。结论HIV低病毒载量人群由于没有得到及时的耐药检测,会仍然沿用当前的治疗方案,不仅治疗没有好的效果,可能对后续的家庭生活和社会效应方面也会有一些负面影响。呼吁加强艾滋病患者治疗前耐药检测,有的放矢地开展HIV抗病毒治疗非常有必要。Objective Drug resistance was analyzed in a portion of low viral load population,to understand the drug resistance of low viral load population in HIV patients.Methods Eighty-seven plasma samples from patients with viral load lower than 1000 copy/ml in 2019—2020 were selected for general epidemiological investigation.HIV viral load detection and HIV-1 genotype resistance detection were completed by in-house method.Drug resistance sequence matching and genotype subtype analysis were completed by using professional software for sequence supplication,analysis and comparison.Results Fourteen sequences were successfully obtained from 87 low viral load populations,of which four patients developed different degrees of drug resistance(drug resistance rate was 4.6%).The analysis of subtypes showed that CRF01-BC subtype was still the main subtype in Xinjiang,but the subtype was complicated,and the main subtypes were CRF01-BC subtype(12 cases,85.7%),B subtype(1 case,7.1%),and CRF01-AE subtype(1 case,7.1%).Conclusion Due to the lack of timely drug resistance detection,people with low viral load of HIV will still use the current treatment regimen,which not only has no good treatment effect,but may also have some negative efifects on the subsequent family life and social effects.It is also very necessary to strengthen the detection of drug resistance in AIDS patients before treatment and carry out targeted HIV antiviral treatment.
