摘  要:目的探讨颈动脉成像应用时间分辨对比剂动态显像技术(TRICKS)与三维时间飞越法磁共振血流成像(3D-TOF-MRA)的优劣性。方法32例行颈部MRA检查患者分别采用TRICKS和3D-TOF-MRA成像。通过对2种方法的颈动脉显示情况进行评分,测算图像的对比噪声比(CNR),比较2种方法在扫描时间、图像质量及CNR的差异。结果2名医师对TRICKS颈动脉成像的图像质量评分分别为(3.03±0.180)分、(3.06±0.181)分,Kappa值为0.87;对颈动脉3D-TOF-MRA的图像质量评分分别为(2.97±0.180)分、(3.00±0.359)分,Kappa值0.75。两组TRICKS及3D-TOF-MRA评分结果的一致性较好,TRICKS图像评分高于3D-TOF-MRA,2种方法的结果差异有统计学意义(Z=3.854,P<0.05)。TRICKS及3D-TOF-MRA的CNR值分别为(13.45±7.75)、(10.12±0.87),差异有统计学意义(t=3.308,P<0.05)。TRICKS及3D-TOF-MRA的扫描时间分别为62.00 s、325.00 s,差异有统计学意义(t=1083.5,P<0.05)。结论TRICKS较3D-TOF-MRA具有更高的时间分辨率,能够清楚显示颈动脉的解剖结构,可作为颈动脉病变筛查的首选检查方法。Objective To investigate the advantages and disadvantages of the application of time-resolved imaging of contrast kinetics sequence(TRICKS)and three dimensional time-of-flight magnetic resonance angiography(3D-TOF-MRA)in carotid artery.Methods 32 patients with cervical MRA were examined by TRICKS and 3D-TOF-MRA.The images of carotid artery display by TRICKS and 3D-TOF-MRA were scored and the contrast noise ratio(CNR)of the images was calculated.Compare the difference of scanning time,image quality and CNR between the two methods.Results The image quality scores of two physicians on TRICKS carotid imaging of carotid artery were(3.03±0.180),(3.06±0.181),the value of kappa was 0.87.The image quality scores of two physicians on 3D-TOF-MRA carotid imaging of carotid artery were(2.97±0.180),(3.00±0.359),the value of kappa was 0.75.The results of the two physicians’scores of TRICKS and 3D-TOF-MRA were in good agreement,and the image scores of TRICKS were higher than those of 3D-TOF-MRA.The differences between the two methods were statistically significant(z=3.854,P<0.05).The CNR of TRICKS and 3D-TOF-MRA were(13.45±7.75)and(10.12±0.87)respectively,which was significantly different(t=3.308,P<0.05).The scanning time of TRICKS and 3D-TOF-MRA were 62.00 s and 325.00 s,respectively,which was significantly different(t=1083.5,P<0.05).Conclusion Compared with 3D-TOF-MRA,TRICKS has a higher rime resolution and can clearly show the anatomical structure of carotid artery.It can be used as the first examination method for carotid artery disease screening.
