职业性石棉所致腹膜间皮瘤1例报道

摘　　要：目的分析职业性石棉所致腹膜间皮瘤患者的临床表现和诊断要点。方法回顾性分析1例女性职业性石棉所致腹膜间皮瘤患者的临床资料和职业病诊断相关情况。结果患者2016年因持续性胸背疼痛、咳嗽、咯泡沫样痰等症状先后在某省多家医院进行诊断和治疗,经腹腔镜手术取腹膜进行病理活检确诊为腹膜间皮瘤;其后于2016年12月因肺部感染导致病情恶化死亡。患者家属于2017年5月要求进行职业病诊断。经当地职业病诊断机构与工业和信息化局调查、核实后明确患者有石棉职业接触史,累计23年2个月。2018年7月被追溯诊断为职业性肿瘤(石棉所致间皮瘤)。结论明确的石棉职业接触史和病理组织学检查有助于职业性肿瘤(石棉所致间皮瘤)的诊断。Objective To analyze the clinical manifestations and diagnostic points of patients with peritoneal mesothelioma caused by occupational asbestos. Methods The clinical data of a female patient with peritoneal mesothelioma caused by occupational asbestos and the diagnosis of occupational diseases were retrospectively analyzed. Results In 2016, the patient was diagnosed and treated in a number of hospitals in a province due to chest and back pain, persistent cough, suffocation, and foamy sputum. After laparoscopic surgery, the peritoneal biopsy was taken for pathological analysis and diagnosed as peritoneal mesothelioma. In December 2016, she died due to a worsening of her condition and lung infection. The patient′s family requested occupational disease diagnosis in May 2017. After investigation and verification by the local occupational disease diagnosis agency and the Bureau of Industry and Information Technology, it was clear that the patient had a history of occupational exposure to asbestos for a total of 23 years and two months. In July 2018, she was retrospectively diagnosed as an occupational tumor(mesothelioma caused by asbestos). Conclusion A clear history of occupational exposure to asbestos and histopathological examination are helpful for the diagnosis of occupational tumors(mesothelioma caused by asbestos).
