摘 要:目的系统评价抗阻运动对糖、脂代谢异常人群(糖尿病前期和2型糖尿病患者)的效果。方法计算机检索PubMed、The Cochrane Library、EMbase、Web of Science、EBSCO、VIP、WanFang Data和CNKI数据库,搜集抗阻运动治疗糖尿病前期和2型糖尿病患者的随机对照试验(RCT),检索时限均为2010年至2021年4月。由2名研究者独立筛选文献、提取资料并评价纳入研究的偏倚风险后,采用RevMan 5.1软件和Stata 12.0软件进行Meta分析。结果共纳入26个RCT,包括2078例患者。Meta分析结果显示:抗阻运动在改善空腹血糖[MD=-0.57,95%CI(-0.69,-0.45),P<0.00001]、糖化血红蛋白[MD=-0.28,95%CI(-0.33,-0.22),P<0.00001]、高密度脂蛋白[MD=0.06,95%CI(0.01,0.11),P=0.01]、低密度脂蛋白[MD=-0.35,95%CI(-0.47,-0.24),P<0.00001]、总胆固醇[MD=-0.25,95%CI(-0.39,-0.12),P=0.0003]、稳态模型-胰岛素抵抗指数[MD=-0.74,95%CI(-0.80,-0.68),P<0.00001]、体质量指数[MD=-0.54,95%CI(-1.03,-0.05),P=0.03]等方面均优于对照组。结论抗阻运动可改善糖、脂代谢异常人群的血糖、血脂和胰岛素抵抗水平。受纳入研究数量和质量的限制,上述结论尚待更多高质量研究予以验证。Objective To systematically review the intervention efficacy of resistance exercise on prediabetes and type 2 diabetes.Methods PubMed,The Cochrane Library,EMbase,Web of Science,EBSCO,VIP,WanFang Data and CNKI databases were electronically searched to collect randomized controlled trials(RCTs)of resistance exercise applied to pre-diabetic and type 2 diabetic population from 2010 to April,2021.Two reviewers independently screened literature,extracted data and assessed the risk of bias of included studies.Meta-analysis was then performed using Revman 5.1 and Stata 12.0 software.Results A total of 26 RCTs involving 2078 patients were included.The results of meta-analysis showed that resistance exercise could improve fasting blood glucose(MD=-0.57,95%CI-0.69 to-0.45,P<0.00001),glycosylated hemoglobin(MD=-0.28,95%CI-0.33 to-0.22,P<0.00001),high density lipoprotein(MD=0.06,95%CI 0.01 to 0.11,P=0.01),low density lipoprotein(MD=-0.35,95%CI-0.47 to-0.24,P<0.00001),total cholesterol(MD=-0.25,95%CI-0.39 to-0.12,P=0.0003),steady-state model-insulin resistance index(MD=-0.74,95%CI-0.80 to-0.68,P<0.00001),and body mass index(MD=-0.54,95%CI-1.03 to-0.05,P=0.03)compared with control group.Conclusions Resistance exercise can improve the blood glucose,blood lipid,and insulin resistance levels of individuals with abnormal glucose and lipid metabolism.Due to limited quality and quantity of the included studies,more highquality studies are required to verify above conclusions.
