摘　　要：目的探讨白细胞介素-17、CD64、CD63对糖尿病足感染程度及溃疡愈合情况的预测价值。方法测定112例糖尿病足合并感染患者(感染组)和112例糖尿病足未合并感染患者(未感染组)血清白细胞介素-17、CD64、CD63水平,比较感染组不同感染程度、溃疡愈合情况患者血清白细胞介素-17、CD64、CD63水平,并分析白细胞介素-17、CD64、CD63与常规感染标志物(白细胞计数、降钙素原、C反应蛋白)的相关性,绘制受试者工作特征曲线探讨其对溃疡愈合不良的预测价值。结果感染组血清白细胞介素-17、CD64、CD63水平显著高于未感染组(P<0.01)。随着感染程度加重患者血清白细胞介素-17、CD64、CD63水平逐渐增高(P<0.01);溃疡愈合不良患者血清白细胞介素-17、CD64、CD63水平显著高于愈合良好者(P<0.01)。合并感染患者血清白细胞介素-17、CD64、CD63水平与血清WBC、PCT、CRP水平呈显著正相关(P<0.01)。受试者工作特征曲线显示,血清白细胞介素-17、CD64、CD63联合预测糖尿病足合并感染患者溃疡愈合不良的曲线下面积为0.807(三者联合>血清白细胞介素-17>血清CD64>血清CD63),灵敏度为75.0%,特异度为78.3%。结论糖尿病足合并感染患者血清白细胞介素-17、CD64、CD63呈过度表达,并随着感染程度增加呈升高趋势,三者联合检测有望成为糖尿病足合并感染患者溃疡愈合的重要预测指标。Objective To investigate the predictive value of interleukin-17(IL-17),CD64 and CD63 on the infection degree of diabetic foot(DF)and ulcer healing.Methods The levels of IL-17,CD64 and CD63 were measured in 112 patients with DF combined infection(infection group)and 112 patients without infection(uninfected group).The levels of serum IL-17,CD64 and CD63 in patients with different degrees of infection and ulcer healing in the infection group were compared,and the correlation between IL-17,CD64 and CD63 and conventional infection markers[white blood cell count(WBC),procalcitonin and C-reactive protein(CRP)]was analyzed.Receiver operating characteristic curve(ROC)was used to explore its predictive value for poor ulcer healing.Results The levels of serum IL-17,CD64 and CD63 in the infection group were significantly higher than those in the uninfected group(P<0.01).With the aggravation of infection,the levels of serum IL-17,CD64 and CD63 increased gradually(P<0.01).The levels of serum IL-17,CD64 and CD63 in patients with poor ulcer healing were significantly higher than those in patients with good ulcer healing(P<0.01).The levels of IL-17,CD64 and CD63 were positively correlated with the levels of WBC,PCT and CRP in patients combined with infection(P<0.01).ROC showed that the area under the curve of poor ulcer healing in patients with DF combined infection predicted by serum IL-17,CD64 and CD63[JP+3]was 0.807(combination of the three>serum IL-17>serum CD64>serum CD63).The sensitivity and specificity were 75.0%and 78.3%respectively.Conclusions Serum IL-17,CD64,and CD63 are over-expressed in patients with DF combined infection,and they increase significantly with the increase of the infection degree,and the combination of the three is expected to be an important predictor of ulcer healing in patients with DF combined infection.
