髋/膝关节置换术证型分布及下肢深静脉血栓辨证预防的病例观察

摘　　要：目的观察髋膝关节置换围手术期患者中医证型的分布及辨证用药预防下肢深静脉血栓形成(DVT)的效果。方法选择行人工髋、膝关节置换患者46例,归纳患者术前后中医证型的分布;患者随机分为试验组与对照组进行DVT预防,比较两组患者DVT发生率。结果试验组无发生DVT;对照组2例患肢肿胀(轻症DVT),发生率8.70%。两组DVT发生率比较,差异有显著性意义(χ^(2)=3.88,P<0.05)。患者术前证型以气滞血瘀证、肝肾亏虚证较多,术后气滞血瘀及气虚血瘀证患者最多,证型分布较为集中(P<0.05)。结论髋膝关节置换患者术后的中医证型分布集中,基于此证型演变规律的中医辨证用药有助于合理预防术后深静脉血栓的发生。Objective To find out the elementary syndrome of patients during hip/knee replacement,and observe the effect of dialectical treatment for preventing deep venous thrombosis.Methods All 46 patients who planned to underwent hip/knee replacements were randomly divided into two groups,experiment group(23 cases were given differential treatment)and controlled group(23 cases were given administration with low molecular heparin).TCM syndrome types were statistically analyzed.Furthermore,clinical preventive effects were compared in the two groups.Results Analyzed from preventive effect of the two groups,there were 2 case of DVT and the incidence rate was 8.70%in controlled group,and the numbers above were respectively no case in experiment group.The curative effect of the two groups were obviously different compared byχ^(2) test(P<0.05).The main pathogens were“qi stagnation and blood stasis syndrome”and“qi deficiency and blood stasis syndrome”after hip/knee replacement.Conclusion The TCM Syndrome of distribution on patients suffered hip/knee replacement usually demonstrate relatively concentrated.Compared with low molecular heparin,differential treatment is effective on preventing DVT after hip/knee replacement.
