基于数据挖掘的癌性发热中医验案的用药规律研究

摘　　要：目的:挖掘癌性发热中医验案的用药规律,指导中医药治疗癌性发热临床用药。方法:检索中国期刊全文数据库、维普数据库、万方数据库,查找癌性发热相关的中医验案,统计验案中所涉及的中药处方,分析药物的使用频次、性味、归经等,给予高频中药聚类及关联分析。结果:纳入文献79篇,中药处方123个。共纳入药物221味,甘草、柴胡、黄芪、生地黄的使用频次较高;清热药占比最大,为23.98%;属寒性的中药最多,占总药味数的51.13%;苦、甘、辛依次是出现频次较高的药味,3药累计药味频率84.66%;归经属肝经者最多,占19.82%。高频中药聚类分析和关联分析,可得到3个聚类方,18对常用药物组合。结论:使用中药治疗癌性发热时,需以清热药、补虚药、化痰止咳平喘药、活血化瘀药为核心用药,根据数据挖掘分析的结果,为癌性发热临床用药给予依据。Objective:To analyze the medication regularity of traditional Chinese medicine cases in treatment of neoplastic fever in order to guide clinical medication.Methods:The database of China National Knowledge Infrastructure,Vip database and WanFang medical network were searched.The cases involving traditional Chinese medicine prescription in the treatment of neoplastic fever were screened.After extracting the prescriptions and Chinese medicines,statistics were made according to frequency analysis,cluster analysis and association analysis.Results:A total of 79 literatures were included,which involved 123 decoctions.221 traditional Chinese medicines were included in the statistics,the most frequently used traditional Chinese medicines were Gancao,Chaihu,Huangqi and Shengdihuang.The proportion of heatclearing drugs in Chinese medicines was the largest(23.98%).The most common property of the Chinese medicines was cold property,accounting for 51.13%.The higher frequencies of flavors of the Chinese medicines were bitter,sweet and acrid,the sum of which was 84.66%.The most common meridian of Chinese medicines was liver meridian,accounting for 19.82%.The cluster analysis and association analysis of high frequency Chinese medicines showed that high frequency Chinese medicines could form 3 clustering formulas,and could get 18 pairs of high frequency drug combinations.Conclusion:Traditional Chinese medicines for heatclearing,deficiencytonifying,phlegmresolving,bloodstasisdispeling are mostly used to the treatment of neoplastic fever.The results of clusteranalysis and association analysis of high frequency traditional Chinese medicines can provide certain reference and guidance for clinical medication.
