摘 要:目的分析红细胞分布宽度(RDW)与原发性高血压患者左心室舒张功能障碍(LVDD)的相关性。方法280例未经治疗的原发性高血压患者,均行超声心动图检查,评价其左心室舒张功能,根据是否合并LVDD将患者分为高血压合并LVDD(LVDD组,98例)与高血压左心室舒张功能正常(Non-LVDD组,182例)。所有患者均查血常规获得RDW。比较两组患者基本资料、实验室指标及心脏彩超指标,分析原发性高血压LVDD的影响因素及独立危险因素。结果Non-LVDD组性别、体质量指数(BMI)、舒张压(DBP)、空腹血糖(FBG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)、血尿酸(UA)及左室射血分数(LVEF)与LVDD组比较,差异无统计学意义(P>0.05);LVDD组年龄、收缩压(SBP)、RDW及二尖瓣舒张早期血流速度(E)值、二尖瓣舒张晚期血流速度(A)值、二尖瓣环舒张早期运动峰速度(E’)值、E峰减速时间(EDT)值、E/A值、E/E’值与Non-LVDD组比较,差异有统计学意义(P<0.05)。Pearson相关性分析显示,年龄、SBP、RDW与E/E’呈正相关(r=0.270、0.237、0.440,P<0.05)。以LVDD为因变量,以年龄、SBP、RDW为自变量,Logistic回归分析显示,年龄、SBP及RDW是高血压合并LVDD的独立危险因素(P<0.05)。结论RDW可作为原发性高血压患者发生LVDD的早期标志物。Objective To analyze the correlation of red cell distribution width(RDW)with left ventricular diastolic dysfunction(LVDD)in patients with essential hypertension.Methods There were 280 patients with untreated essential hypertension,and all patients underwent echocardiography to evaluate their left ventricular diastolic function.According to whether they were combined with LVDD,they were divided into those with hypertension and LVDD(LVDD group,98 cases)and those with hypertension and normal left ventricular diastolic function(Non-LVDD group,182 cases).All patients obtained RDW by blood routine examination.The basic data,laboratory indicators and cardiac color Doppler ultrasound indicators were compared between the two groups,and the influencing factors and independent risk factors of LVDD in patients with essential hypertension were analyzed.Results There were no statistically significant differences in gender,body mass index(BMI),diastolic blood pressure(DBP),fasting blood glucose(FBG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),uric acid(UA)and left ventricular ejection fraction(LVEF)in the Non-LVDD group compared with the LVDD group(P>0.05).The differences in age,systolic blood pressure(SBP),RDW and early diastolic peak flow velocity of mitral valve(E),late diastolic flow velocity of mitral valve(A),early diastolic peak flow velocity of mitral valve(E’),E peak deceleration time(EDT),E/A,E/E’between LVDD group and Non-LVDD group were statistically significant(P<0.05).Pearson correlation analysis showed that age,SBP,and RDW were positively correlated with E/E’(r=0.270,0.237,0.440,P<0.05).Logistic regression analysis showed that age,SBP,and RDW were independent risk factors for hypertension combined with LVDD,with LVDD as the dependent variable and age,SBP,and RDW as independent variables(P<0.05).Conclusion RDW can be used as an early marker for the development of LVDD in patients with essential hypertension.
