沙丁胺醇联合丙酸倍氯米松治疗成人哮喘的效果分析

摘　　要：目的探讨沙丁胺醇联合丙酸倍氯米松治疗成人哮喘的临床价值。方法90例成人哮喘患者,随机分为对照组与干预组,各45例。对照组给予单一沙丁胺醇雾化吸入治疗,研究组给予沙丁胺醇联合丙酸倍氯米松雾化吸入治疗。对比两组治疗效果、肺功能指标、炎性因子指标、血气指标。结果干预组治疗总有效率95.6%显著高于对照组的82.2%,差异有统计学意义(P<0.05)。治疗后,干预组呼气峰值流速(PEF)、用力肺活量(FVC)、第1秒用力呼气容积(FEV1)水平分别为(189.44±0.65)L/min、(2.56±0.66)L、(2.46±0.27)L,均优于对照组的(178.53±0.21)L/min、(2.11±0.24)L、(2.03±0.31)L,差异均有统计学意义(P<0.05)。治疗后,干预组C反应蛋白(CRP)、白细胞介素-8(IL-8)、肿瘤坏死因子-α(TNF-α)水平分别为(2.27±0.37)mg/L、(14.53±1.54)ng/L、(0.25±0.14)μg/L,均优于对照组的(6.23±0.61)mg/L、(17.22±1.88)ng/L、(0.54±0.17)μg/L,差异均有统计学意义(P<0.05)。治疗后,干预组动脉血氧分压(PaO_(2))、动脉血二氧化碳分压(PaCO_(2))、血氧饱和度(SaO_(2))水平分别为(80.45±3.12)mm Hg(1 mm Hg=0.133 kPa)、(38.34±2.51)mm Hg、(94.63±1.25)%,均优于对照组的(74.87±3.04)mm Hg、(43.41±3.14)mm Hg、(91.44±1.21)%,差异均有统计学意义(P<0.05)。结论治疗成人哮喘联合使用沙丁胺醇与丙酸倍氯米松雾化吸入治疗可改善其肺功能与血气指标,消除炎性因子,进一步提升临床疗效,建议推广应用。Objective To discuss the clinical value of salbutamol combined with beclomethasone dipropionate in the treatment of adult asthma.Methods A total of 90 adults with asthma were randomly divided into the control group and the intervention group,with 45 cases in each group.The control group was treated with salbutamol aerosol inhalation,and the intervention group was treated with salbutamol and beclomethasone dipropionate aerosol inhalation.The therapeutic effect,lung function index,inflammatory factor index and blood gas index were compared between the two groups.Results The total effective rate of treatment 95.6%in the intervention group was significantly higher than 82.2%in the control group,and the difference was statistically significant(P<0.05).After treatment,the peak expiratory flow(PEF),forced vital capacity(FVC),and forced expiratory volume in the 1st second(FEV1)of the intervention group were(189.44±0.65)L/min,(2.56±0.66)L,and(2.46±0.27)L,which were all better than(178.53±0.21)L/min,(2.11±0.24)L,and(2.03±0.31)L of the control group,and the difference was statistically significant(P<0.05).After treatment,the levels of C-reactive protein(CRP),interleukin-8(IL-8)and tumor necrosis factor-α(TNF-α)in the intervention group were(2.27±0.37)mg/L and(14.53±1.54)ng/L,(0.25±0.14)μg/L,which were better than(6.23±0.61)mg/L,(17.22±1.88)ng/L,(0.54±0.17)μg/L in the control group,and the differences were statistically significant(P<0.05).After treatment,the arterial partial pressure of oxygen(PaO_(2)),arterial partial pressure of carbon dioxide(PaCO_(2)),and blood oxygen saturation(SaO_(2))of the intervention group were(80.45±3.12)mm Hg(1 mm Hg=0.133 kPa),(38.34±2.51)mm Hg,(94.63±1.25)%,which were better than(74.87±3.04)mm Hg,(43.41±3.14)mm Hg,(91.44±1.21)%of the control group,and the differences were statistically significant(P<0.05).Conclusion Aaerosol inhalation with salbutamol and beclomethasone dipropionate for adult asthma can improve the lung function and blood gas index,eliminate inflammatory
