右美托咪定联合瑞芬太尼在严重烧伤休克早期患者清创手术中的应用研究

摘　　要：目的探讨右美托咪定联合瑞芬太尼在严重烧伤休克早期患者清创手术中的应用。方法选取2019年3月~2021年3月接受清创手术治疗的150例严重烧伤休克早期患者为研究对象,根据麻醉用药不同,将患者随机分为观察组(n=75)和对照组(n=75),对照组应用丙泊酚联合瑞芬太尼,观察组应用右美托咪定联合瑞芬太尼,对比两组患者平均动脉压、心率、皮质醇水平、血糖水平。结果比较两组患者给药后各项指标:两组平均动脉压均有下降趋势,观察组平均动脉压明显低于对照组(P<0.05);两组心率均有所下降,观察组心率明显低于对照组(P<0.05);两组皮质醇水平均有所上升,观察组明显低于对照组(P<0.05);两组血糖水平均有所上升,观察组明显低于对照组(P<0.05)。结论右美托咪定联合瑞芬太尼应用于严重烧伤休克早期患者清创手术中,利于血流动力学稳定,减轻应激反应。Objective To explore the application of dexmedetomidine combined with remifentanil in debridement of patients with severe burn shock in the early stage.Methods 150 patients with severe burns in early shock who underwent debridement surgery in our hospital from March 2019 to March 2021 were selected as the research objects.According to different anesthesia medications,the patients were randomly divided into the observation group(n=75)and the control group(n=75).The control group was treated with propofol combined with remifentanil,and the observation group was treated with dexmedetomidine combined with remifentanil.The average arterial pressure,heart rate,cortisol level,and blood glucose level of the two groups were compared.Results Comparison of the indicators of the two groups of patients after administration:the average arterial pressure of the two groups had a decreasing trend,the average arterial pressure of the observation group was significantly higher than that of the control group(P<0.05);the heart rate of both groups decreased,and the heart rate of the observation group was significantly lower than the control group(P<0.05);the cortisol levels of the two groups increased,and the observation group was significantly lower than the control group(P<0.05);the blood glucose levels of the two groups increased,and the observation group was significantly lower than the control group(P<0.05).Conclusion The combination of dexmedetomidine and remifentanil in debridement of patients with severe burn shock in the early stage is beneficial to hemodynamic stability and reducing stress response.
