环孢素血药浓度水平对慢性再生障碍性贫血免疫抑制治疗近期疗效的影响研究

摘　　要：目的分析环孢素(CsA)血药浓度水平对接受免疫抑制治疗(IST)慢性再生障碍贫血(CAA)患者近期疗效的影响。方法回顾性分析2017年1月~2020年12月收治采取IST的CAA患者共40例的临床资料,对比IST后6个月未获治疗反应和获治疗反应者之间的CsA血药浓度值差异,不同CsA血药浓度的CAA患者治疗反应率。结果IST第1个月、第2个月以及第4~6个月未获治疗反应组和获治疗反应组CsA血药浓度C_(0)以及C_(2)值差异无统计学意义(P>0.05);IST第3个月,获治疗反应组CsA血药浓度C_(0)及C_(2)值高于未获治疗反应组(P<0.05)。IST第3个月,C_(0)值≥200μg/L组治疗反应率高于C_(0)值150~200μg/L、C_(0)值<150μg/L组(P<0.05);C_(2)≥700μg/L组治疗反应率高于C_(2)<700μg/L组(P<0.05)。结论IST第3个月时的CsA血药浓度值对患者获取早期疗效有影响,控制CsA血药浓度值C_(0)≥200μg/L与C_(2)≥700μg/L能提升治疗反应率。Objective To analyze the short-term effects of cyclosporine A(CsA)blood concentrations in chronic aplastic anemia(CAA)patients using immunosuppressive therapy(IST).Methods Clinical data about 40 CAA patients of undergoing IST(January 2017 to December 2020)was retrospectively analyzed.After 6 m of IST,CsA blood concentrations between response patients and non-response patients were compared.The treatment response rates among CAA patients with different CsA blood concentrations were recorded.Results Upon first,second and fourth-sixth month of IST,CsA blood concentrations C_(0) and C_(2) values between response group and non-response group were not significantly different(P>0.05);upon third month of IST,CsA blood concentrations C_(0) and C_(2) values in response group were higher than non-response group(P<0.05);upon third month of IST,the treatment response rates in C_(0)≥200μg/L group were higher than C_(0)=150~200μg/L group and C_(0)<150μg/L group(P<0.05);the treatment response rates in C_(2)≥700μg/L group were higher than C_(2)<700μg/L group(P<0.05).Conclusion Upon third month of IST,CsA blood concentrations produce influences in the early treatment effects of CAA patients.Therefore,the treatment response rates may be increased by controlling CsA blood concentrations C_(0)≥200μg/L and C_(2)≥700μg/L.
