DRGs与传统指标在临床科室医疗服务能力评价中的应用

摘　　要：目的研究DRGs指标与传统指标在综合评价临床科室医疗服务能力方面的应用。方法回顾性分析某三甲医院临床科室2018年1—12月的出院病案首页资料,通过熵权秩和比法对临床38个科室进行DRGs指标和传统指标的综合评价和比较,分析评价中的排序结果。结果DRGs评价指标与传统指标得到的评价结果存在一定差异,DRGs各指标综合评价结果居前3位的是科室1、科室2和科室3,秩和比值分别是0.1784、0.1602、0.1520。通过传统指标综合评价结果排名前3的分别是科室3、科室12和科室9,秩和比值分别是0.1639、0.1579、0.1566。DRGs指标评价排名前十位的科室中通过传统指标评价仅有4个科室。结论通过DRGs评价指标进行科室间的综合比较更加符合实际,今后可作为临床科室评价的有效方法。Objective To study the application of DRGs indicators and traditional indicators in the comprehensive evaluation of the medical service capabilities of clinical departments.Methods Retrospectively analyze the first page data of the discharged medical records of the clinical department of a tertiary hospital from January to December 2018,and conduct a comprehensive evaluation and comparison of DRGs indicators and traditional indicators in 38 clinical departments through the entropy-weighted rank sum ratio method,and analyze the ranking results in the evaluation.Results There was a certain difference between DRGs evaluation index and the evaluation result obtained by traditional index.The top three comprehensive evaluation results of DRGs index were Department 1,Department 2,and Department 3.The rank sum ratios were 0.1784,0.1602,and 0.1520,respectively.According to the comprehensive evaluation results of traditional indicators,the top three rankings were Section 3,Section 12 and Section 9,respectively.The rank sum ratios were 0.1639,0.1579,and 0.1566,respectively.Among the top ten departments in DRGs index evaluation,there were only 4 departments evaluated by traditional indicators.Conclusion Comprehensive comparison between departments through DRGs evaluation indicators is more realistic,and it can be used as an effective method for clinical department evaluation in the future.
