摘 要:目的探讨去铁酮联合去铁胺治疗重型地中海贫血(海洋性贫血)患儿的效果。方法方便选取该院儿童治疗中心于2017年7月—2019年3月收治的61例重型地中海贫血患儿的资料进行分析,按照投掷硬币法将其分为对照组(30例)和观察组(31例),对照组患儿注射用甲磺酸去铁胺静滴,观察组在此基础上加用去铁酮片口服,比较两组患儿的临床疗效、血清铁蛋白和尿铁排泄量、血糖以及不良反应情况。结果观察组总有效率80.64%明显高于对照组53.33%,差异有统计学意义(χ^(2)=5.161,P<0.05)。治疗后,观察组血清铁蛋白(1852.32±287.07)ng/mL明显低于对照组,观察组尿铁排泄量(0.77±0.09)ng/mL明显高于对照组,差异有统计学意义(t=5.747、4.655,P<0.05)。观察组血糖正常率90.32%明显高于对照组50.00%,差异有统计学意义(χ^(2)=11.917,P<0.05)。对照组不良反应发生率26.67%与观察组29.03%比较,差异无统计学意义(P>0.05)。结论去铁酮联合去铁胺治疗重型地中海贫血患儿具有更好的临床效果,可以降低血清铁蛋白,减少尿铁排泄量,稳定血糖且不增加不良反应发生率。Objective To evaluate the effect of deferiprone combined with deferoxamine in children with thalassemia major(thalassemia major).Methods Conveniently selected the data of 61 children with thalassemia major admitted to the children's treatment center of this hospital from July 2017 to March 2019 were analyzed.According to the coin tossing method,they were divided into a control group(30 cases)and an observation group(31 cases).In the control group,deferoxamine mesylate was injected intravenously,and the observation group was taken orally with deferiprone tablets on this basis.The clinical efficacy,serum ferritin and urinary iron excretion,blood glucose and adverse reactions of the two groups were compared.Results The total effective rate of 80.64%in the observation group was significantly higher than 53.33%in the control group,and the difference was statistically significant(χ^(2)=5.161,P<0.05).After treatment,the serum ferritin(1852.32±287.07)ng/mL of the observation group was significantly lower than that of the control group.The urinary iron excretion(0.77±0.09)ng/mL of the observation group was significantly higher than that of the control group.The difference was statistically significant(t=5.747,4.655,P<0.05).The normal blood glucose rate of the observation group was 90.32%significantly higher than that of the control group 50.00%,and the difference was statistically significant(χ^(2)=11.917,P<0.05).The incidence of adverse reactions in the control group was 26.67%compared with 29.03%in the observation group,and the difference was not statistically significant(P>0.05).Conclusion The combination of deferiprone and deferoxamine has a better clinical effect in the treatment of children with thalassemia major.It can reduce serum ferritin,reduce urinary iron excretion,stabilize blood glucose without increasing the incidence of adverse reactions.
