摘　　要：胎儿炎症反应综合征(FIRS)是一种由胎儿的全身炎症、促炎细胞因子迅速增加进入胎儿循环(包括白细胞介素-1和白细胞介素6)及细胞反应(例如中性粒细胞、单核细胞/巨噬细胞和T细胞增加)和存在脐带炎所定义的疾病。胎儿炎症反应综合征的特征在于脐带炎症和胎儿促炎细胞因子升高。FIRS可能导致胎儿和新生儿死亡并损伤多系统器官,其中最严重的长期并发症是脑损伤及随后发生的脑瘫和认知障碍。新生儿脑病(NE)是新生儿重症监护病房中的一种常见疾病。多种原因可引起新生儿脑病,包括缺氧缺血性脑病(HIE)和胎儿炎症反应综合征。其中,FIRS与新生儿脑病的发生密切相关。该文就近年FIRS及新生儿脑病的临床诊断、胎儿炎症反应综合征致新生儿脑损伤的发病机制等方面的研究进展进行综述。Fetal inflammatory response syndrome(FIRS)is defined by systemic inflammation of the fetus,a rapid increase in proinflammatory cytokines into the fetal circulation(including interleukin-1 and interleukin-6),and cellular responses(e.g.increased neutrophils,monocytes/macrophages,and T cells)and the presence of umbilical cord inflammation.Fetal inflammatory response syndrome is characterized by umbilical cord inflammation and elevated fetal proinflammatory cytokines.FIRS can cause fetal and neonatal death and damage multi-system organs,with the most serious long-term complications being brain damage and subsequent cerebral palsy and cognitive impairment.Neonatal encephalopathy(NE)is a common disease in neonatal intensive care unit.Neonatal encephalopathy can be caused by a variety of causes,including hypoxic ischemic encephalopathy(HIE)and fetal inflammatory response syndrome.Among them,FIRS is closely related to the occurrence of neonatal encephalopathy.In this review,this artical review the recent advances in the clinical diagnosis of FIRS and neonatal encephalopathy,and the pathogenesis of neonatal brain injury caused by fetal inflammatory response syndrome.
