改良混合痔外切内扎术联合内痔硬化剂注射治疗环状混合痔疗效分析

摘　　要：目的探讨改良混合痔外切内扎术联合内痔硬化剂注射治疗环状混合痔的临床疗效和安全性。方法于2017年1月—2019年12月方便选取苏州市相城区中医医院肛肠科收治的环状混合痔患者98例。采用随机数字分组法将研究对象分为观察组(n=49)与对照组(n=49)。观察组采用改良内痔结扎+外痔切除术联合聚桂醇注射治疗,对照组采用传统外剥内扎术治疗。分别记录两组患者的术中出血量、手术时间以及术后住院时间。观察两组术后并发症(大便难解、肛门狭窄、肛门水肿、尿潴留)的发生情况以及治疗有效率。结果观察组术中出血量少于对照组[(7.66±2.95)mL vs(9.27±3.72)mL],术后住院时间短于对照组[(6.82±1.26)d vs(7.71±1.47)d],差异有统计学意义(t=2.374、3.218,P<0.05)。观察组治疗有效率97.96%高于对照组81.63%,术后并发症发生率6.12%低于对照组22.45%,差异有统计学意义(χ^(2)=7.127、5.333,P<0.05)。结论改良内痔结扎+外痔切除术联合聚桂醇注射治疗环状混合痔疗效满意,术后创面愈合速度快,并且术后并发症发生率低。Objective To investigate the clinical efficacy and safety of modified mixed hemorrhoids external resection and internal ligation combined with internal hemorrhoid sclerosing agent injection in the treatment of circular mixed hemorrhoids.Methods 98 patients with circular mixed hemorrhoids admitted to the department of anorectal,Xiangcheng district Chinese Medicine hospital,Suzhou city from January 2017 to December 2019 were conveniently selected.The study subjects were divided into observation group(n=49)and control group(n=49)by random number grouping method.The observation group was treated with modified internal hemorrhoid ligation+external hemorrhoidectomy combined with cinnamyl alcohol injection,and the control group was treated with traditional external stripping and internal ligation.The intraoperative blood loss,operation time and postoperative hospital stay of the two groups of patients were recorded.The occurrence of postoperative complications(difficult stool,anal stenosis,anal edema,and urinary retention)and the effective rate of treatment were observed in the two groups.Results The intraoperative blood loss of the observation group was less than that of the control group[(7.66±2.95)mL vs(9.27±3.72)mL],and the postoperative hospital stay was shorter than that of the control group[(6.82±1.26)d vs(7.71±1.47)d],the difference was statistically significant(t=2.374,3.218;P<0.05).The effective rate of treatment in the observation group 97.96%was higher than that of the control group 81.63%,and the postoperative complication rate 6.12%was lower than that of the control group 22.45%.The difference was statistically significant(χ^(2)=7.127,5.333;P<0.05).Conclusion Modified internal hemorrhoid ligation+external hemorrhoidectomy combined with cinnamyl alcohol injection in the treatment of circular mixed hemorrhoids has a satisfactory effect,the wound healed quickly after surgery,and the incidence of postoperative complications was low.
