摘　　要：目的分析慢性鼻-鼻窦炎患者手术治疗基础上联合激素治疗与维生素D对患者鼻腔黏膜恢复以及肿瘤坏死因子α、C反应蛋白水平变化。方法方便选取该院2020年1—12月收治的慢性鼻-鼻窦炎患者90例,按不同的治疗方式分组,单纯组(n=45)采用鼻窦内窥镜手术结合激素治疗,联合组(n=45)采用鼻窦内窥镜手术结合维生素D治疗,比较两组患者的临床治疗疗效、并发症发生率以及术前、术后3个月的鼻腔黏膜恢复情况、炎症因子水平。结果联合组临床治疗总有效率为93.33%明显比单纯组77.78%高,并发症发生率为8.88%,明显比单纯组24.44%低,差异有统计学意义(χ^(2)=4.406、3.920,P<0.05)。两组治疗前的鼻腔黏膜恢复评分、炎症因子水平比较,差异无统计学意义(P>0.05);治疗3个月后联合组鼻腔黏膜恢复评分以及肿瘤坏死因子(TNF-α)、C反应蛋白(CRP)指标与单纯组对比,差异有统计学意义(P<0.05)。结论采用手术联合激素治疗、维生素D治疗慢性鼻-鼻窦炎患者,能显著改善患者的病症,提高治疗效果,值得临床应用。Objective Analysis of chronic rhinosinusitis patients combined with hormone therapy and vitamin D on the basis of surgical treatment of patients with nasal mucosa recovery and tumor necrosis factor alpha,C-reactive protein levels.Methods Conveniently selected 90 patients with chronic rhinosinusitis in the hospital from January to December 2020.According to different treatment methods,the simple group(n=45)received endoscopic sinus surgery combined with hormone therapy,and the combined group(n=45)received endoscopic sinus surgery combined with vitamin D treatment.The clinical treatment efficacy,complication rate,nasal mucosal recovery and inflammatory factor levels were compared between the two groups of patients before and 3 months after surgery.Results The total effective rate of clinical treatment in the combined group was 93.33%which was higher than that of the simple group 77.78%,and the complication rate was 8.88%,which was lower than that of the simple group,24.44%the difference was statistically significant(χ^(2)=4.406,3.920,P<0.05).There was no significant difference in the nasal mucosal recovery score and the level of inflammatory factors between the two groups before treatment(P>0.05).After 3 months of treatment,the nasal mucosal recovery score and tumor necrosis factor(TNF-α)and c-reactive protein(CRP)indexes of the combined group were compared with those of the simple group,the difference was statistically significant(P<0.05).Conclusion The use of surgery combined with hormone therapy and vitamin D to treat patients with chronic rhinosinusitis can significantly improve the patient's symptoms and improve the therapeutic effect.It is worthy of clinical application.
