针刺劳宫穴对中医烦躁焦虑状态fMRI脑功能研究

摘　　要：目的观察针刺劳宫穴(PC8)对中医烦躁焦虑状态脑功能的调节。方法招募山东中医药大学硕士研究生中医烦躁焦虑状态受试者24例,随机平均分成针刺组和非针刺组,每组各12例,针刺组于针刺劳宫穴后脑功能磁共振成像(blood level develop-functional magnetic resonance imaging,BOLD-fMRI)扫描,非针刺组于非针刺状态下行BOLD-fMRI扫描,采用低频振幅(the amplitudeof low-frequency fluctuations,ALFF)寻找2组差异脑区,并以各差异脑区为感兴趣区分析与全脑功能连接(functional connectivity,FC)的改变。结果针刺劳宫穴组与非针刺组比较,左侧海马旁回、梭状回、右侧颞上回激活,右侧额中回、右侧楔前叶、楔叶功能抑制;双侧小脑半球、梭状回与右侧海马、海马旁回、右侧中央后回、右侧角回FC减弱,双侧腹内侧前额叶与右侧小脑半球、左侧海马、海马旁回、左侧颞上回FC增强,双侧楔叶与双侧眶额回、右侧顶下小叶FC减弱。结论针刺劳宫穴可以调节中医烦躁焦虑状态患者的脑功能。Objective To observe the regulation of acupuncture at laogong acupoint(PC8)on the brain function of TCM irritability and anxiety state(IAS).Methods From August 2019 to August 2020,24 subjects with irritability and anxiety recruited from graduate students of Shandong University of Traditional Chinese Medicine were randomly divided into acupuncture group and non-acupuncture group,with 12 people in each group.The control group received brain functional MRI scan,the acupuncture group received fMRI(blood level develop-functional magnetic resonance imaging,BOLD-fMRI,fMRI)scan after acupuncture at laogong acupoint(PC8).We used ALLF to find different brain areas,and then analyzed the changes in whole brain FC by using each difference as interest area.Results All subjects were right-handed.There were 6 males and 6 females in the acupuncture group,aged 23±2 years old,with a score greater than 84 on the traditional chinese medicine anxiety and anxiety status rating scale,and the case was same in the non-acupuncture group.The brain function activation area of the blank group was the right thalamus and middle frontal gyrus,and the negative brain function activation area was the left middle frontal gyrus;after the acupuncture group acupunctured Laogong,the fMRI showed that the peak of activation or negative activation was the center in the irregular clusters.The left parahippocampal gyrus,fusiform gyrus,and right superior temporal gyrus were activated;the right middle frontal gyrus,right precuneus,and cuneiform lobe function were inhibited.The specific brain area activated by acupuncture at Laogong acupoint was closely related to the treatment range of the main disease.Conclusion Acupuncture at Laogong acupoint can regulate the brain function of patients with irritability and anxiety in traditional Chinese medicine.
