摘 要:目的观察特发性膜性肾病(IMN)中足细胞的超微结构改变,探讨IMN发病机制。方法对解放军联勤保障部队第900医院2014年6月至2017年9月已确诊并有完整临床资料的53例IMN进行回顾性病例对照研究,通过透射电镜(TEM)观察足细胞的形态学变化,探讨其发病机制。结果在TEM下,IMN的足细胞均有不同程度的足突融合/微绒毛变性、自噬溶酶体增多、线粒体肿胀、脂滴/蛋白滴及细胞膜破损。结论足细胞受损可作为区分特发性膜性肾病和继发性膜性肾病的一个诊断依据。Objective To observa the ultrastructural changes of podocytes in idiopathic membranous nephropathy(IMN)and explore the pathogenesis of IMN.Methods 53 cases of IMN with complete clinical data diagnosed in the900 th Hospital of the joint logistics support force of the PLA from June 2014 to September 2017 were studied by transmission electron microscope(TEM)to observe the morphological changes of podocytes and explore its pathogenesis.Results with the podocyte fusion/microvilli,mitochondrial swelling,vacuolar degeneration,lipid droplets/protein drop and cell membrane damage were obvious under TEM.Conclusion podocyte damage can be used as a diagnostic element to distinguish idiopathic membranous nephropathy from secondary membranous nephropathy.
