成都某社区中老年男性迟发性性腺功能减退症的调查研究

摘　　要：目的了解成都某社区中老年男性迟发性性腺功能减退症(LOH)的发病情况。方法2019年5—8月成都市温江某社区固定人群中采用年龄分层抽样选取1000名30~70岁的中老年男性,进行中老年男性症状问卷(AMS)调查,同时测定血清中总睾酮(TT)水平。结果AMS、TT、AMS联合TT筛查LOH阳性率分别为29.09%、18.10%、12.33%;AMS、TT、AMS+TT筛查LOH阳性率在各年龄组之间比较,差异有统计学意义(χ^(2)=31.216、14.143、8.577,P<0.05)。AMS量表、AMS量表联合血清TT筛查阳性情况与年龄之间均呈正相关(r=0.164、0.161,P<0.05);AMS、TT、AMS联合TT筛查LOH的最佳年龄切点分别为41.5岁、50.5岁、57.0岁。结论成都某社区男性迟发性性腺功能减退症的筛查阳性率较高,并与年龄有一定的相关性,其诊断年龄切点为40~60岁左右,但仍有待进一步验证。Objective To understand the incidence of late-onset hypogonadism(LOH)in middle-aged and elderly men in a community in Chengdu.Methods From May to August 2019,1000 middle-aged and elderly men aged 30-70 were selected by age stratified sampling from a fixed population in a community in Wenjiang,Chengdu.Aging Males'Symptomsscale(AMS)was conducted,and the level of total testosterone(TT)in serum was measured.Results The positive rates of AMS,TT,and AMS combined with TT screening for LOH were 29.09%,18.10%,and 12.33%,respectively.The positive rates of LOH screened by AMS,TT and AMS+TT were compared among different age groups,and the difference was statistically significant(χ^(2)=31.216,14.143,8.577,P<0.05).There was a positive correlation between AMS scale,AMS scale combined with serum TT screening positive status and age(r=0.164,0.161,P<0.05);the best age cut-off points for AMS,TT,AMS combined with TT screening for LOH were respectively 41.5 years old,50.5 years old,and 57.0 years old.Conclusion The positive rate of screening for late-onset hypogonadism in men in a certain community in Chengdu is relatively high,and it has a certain correlation with age.The cut-off point of diagnosis is about 40-60 years old,but it still needs further verification.
