摘　　要：目的探讨经阴道子宫输卵管四维超声造影(TVS 4D-Hycosy)操作过程中常见问题及对策。方法选择本院2017年12月—2019年7月行TVS 4D-Hycosy患者1348例,探讨操作过程中的常见问题、原因及处理方法。结果常见问题包括疼痛(65.96%)、造影剂逆流(23.61%)、迷走神经反射(4.87%)、插管困难(2.15%)及子宫或输卵管显像不全(1.12%)等。结论TVS 4D-Hycosy操作过程中常见问题多可在术前积极预测及规避,术前全面评估、剔除不宜4D-Hycosy的患者,术中积极沟通及心理疏导等对策可以对造影的顺利进行起到良好的效果。Objective To examine the common problems during operation of 4D Hysterosalpingo-contrast sonography(4D-Hycosy)and evaluate the treatment strategy.Methods Totally 1348 patients undergoing 4D-Hycosy from December 2017 to July 2019 were selected in our hospital.Problems during the operation were discussed as well as the causes and solutions.Results Among all the complications,pevic pain was experienced by 65.96%of subjects,followed by intravasation(23.61%),vaso-vagal reactions(4.87%),failure of catheter-insertion(2.15%)and incomplete image of uterine or oviduct(1.12%).Conclusion Common problems during the operation of 4D-Hycosy can be predicted and avoided actively before surgery.Preoperative comprehensive evaluation,elimination of patients not suitable for 4D-Hycosy,and positive intraoperative communication and psychological counseling can play a good role in the smooth operation.
