妊娠期糖尿病患者与正常妊娠妇女幽门螺杆菌感染等多因素对比分析研究

摘　　要：目的探究妊娠期糖尿病(GDM)孕妇的幽门螺杆菌(Hp)水平变化,分析妊娠期糖尿病与Hp感染的关系。方法选取2020年2—8月孕周在24~28周产前检查确诊为GDM的患者120例为GDM组,同期健康孕妇120名为对照组。采用Logistic回归,以是否患妊娠期糖尿病为因变量,是否HP感染、产妇孕前年龄、孕周、是否吸烟、孕前BMI等为自变量构建Logistic回归模型,筛选妊娠期糖尿病的独立影响因素。结果 GDM组Hp感染的阳性率(37.50%)高于对照组(17.50%),差异有统计学意义(P<0.05)。年龄31~35岁、36~40岁、孕前BMI 24~27.9 kg/m^(2)、孕前BMI≥28 kg/m^(2)、Hp感染为GDM的危险因素(OR_(调整后)=1.54,95%CI=1.19~3.32,P<0.001;OR_(调整后)=2.17,95%CI=1.24~3.88,P<0.001;OR_(调整后)=2.11,95%CI=1.33~4.12,P<0.001;OR_(调整后)=2.76,95%CI=1.78~5.32,P<0.001;OR_(调整后)=2.62,95%CI=1.67~3.95,P=0.005)。结论孕前年龄、孕前BMI、幽门螺杆菌感染是妊娠期糖尿病的独立危险因素。Objective To explore the changes in Helicobacter pylori(Hp) levels of pregnant women with gestational diabetes(GDM), and analyze the relationship between gestational diabetes and Hp infection. Methods From February 2020 to August 2020, 120 patients diagnosed with GDM during gestational weeks 24 to 28 weeks before delivery were collected as the GDM group, and 120 healthy pregnant women were the control group during the same period. Logistic regression was used to construct a logistic regression model with whether gestational diabetes was the dependent variable, Hp infection,maternal age before pregnancy, gestational age, smoking, pre-pregnancy BMI, etc. as independent variables to construct a Logistic regression model to screen independent influencing factors of gestational diabetes. Results The positive rate of Hp infection in GDM group(37.50%) was higher than that in control group(17.50%), and the difference was statistically significant(P <0.05). Age 31-35 years, 36-40 years, BMI 24-27.9 kg/m^(2) before pregnancy, BMI ≥28 kg/m^(2) before pregnancy, Hp infection were risk factors for GDM(OR_(adjusted)=1.54, 95%CI=1.19-3.32, P<0.001;OR_(adjusted)=2.17, 95%CI=1.24-3.88, P<0.001;OR_(adjusted)=2.11, 95%CI= 1.33-4.12, P<0.001;OR_(adjusted)=2.76, 95%CI= 1.78-5.32, P<0.001;OR_(adjusted)=2.62, 95%CI=1.67-3.95, P =0.005). Conclusion Age before pregnancy, BMI before pregnancy, and Helicobacter pylori infection are independent risk factors for gestational diabetes.
