摘  要:目的探讨应用骨折风险评估工具(FRAX)预测老年人骨密度(BMD)的可靠性。方法对2018—2020年在福建省老年医院住院的老年患者进行BMD检查的205例患者开展问卷调查,用FRAX工具计算不代入BMD值的研究对象10年内主要骨质疏松性骨折预测值(PMOF),并结合BMD结果统计分析。结果 (1)骨量异常组:患者年龄较大、女性比例较高、身高较矮、体重较轻;(2)以DXA检查结果为诊断骨质疏松的金标准,FRAX评分(PMOF)预测骨密度的ROC曲线下面积为0.890(0.843~0.937),最佳诊断截点(cutoff值)为3.24%。(3)以PMOF值3.24%预测骨量异常的敏感性为85.0%、特异性为88.2%。结论 FRAX结果预测骨密度异常的敏感性(85.0%)与特异性(88.2%)较高,可作为基本筛查工具,以识别老年人骨密度异常的高危人群。Objective To explore the reliability of prediction of bone mineral density(BMD)in the elderly by FRAX tool. Methods Totally 205 elderly patients in Fujian Geriatric Hospital with BMD examination from 2018 to 2020 were investigated by questionnaire. The values of probability of major osteoporotic fracture(PMOF) in 10 years without BMD examination were calculated by using FRAX tool and analyzed with BMD examination results. Results(1)Abnormal bone mass group: the age of the patients was older, the percentage of females higher, the height shorter, and the body weight lower.(2)DXA test results were taken as the gold standard for diagnosis of osteoporosis, the area under ROC curve of BONE mineral density predicted by FRAX score(PMOF) was 0.890(0.843-0.937), and the best cutoff value was 3.24%.(3)When using 3.24% of PMOF value as cut-off point for predicting bone mass abnormality, the sensitivity and specificity were 85.0% and 88.2%, respectively. Conclusion The FRAX tool have high sensitivity(85.0%) and specificity(88.2%) for predicting abnormal bone mineral density, and can be used as a basic screening tool to identify high-risk populations with abnormal bone mineral density in the elderly.
