摘 要:目的比较胶体金法、酶联免疫法和化学发光法检测新型冠状病毒(2019-nCoV)总抗体的应用价值。方法以2020年2-3月昆明市第三人民医院门诊及住院确诊的新型冠状病毒肺炎(COVID-19)患者39例为观察对象,收集不同发病时间空腹静脉血,分别采用胶体金法、酶联免疫法和化学发光法检测样本2019-nCoV总抗体水平并进行统计分析。结果胶体金法、酶联免疫法和化学发光法的灵敏度分别为85.71%、86.81%和81.32%,特异度分别为99.21%、95.24%和97.22%,受试者工作特征曲线(ROC curve)下面积(AUC)分别为0.925、0.938和0.965。此3种方法在机体发病不同时间对2019-nCoV抗体检出率差异无统计学意义;随着发病时间延长,3种方法对2019-nCoV抗体的检出率呈先升高后下降的趋势(检出率在发病后第15~21 d达峰)。结论化学发光法诊断COVID-19的准确性最高,此3种方法的诊断性能均能满足临床需求。Objective To compare the clinical application value of three 2019-nCoV Total Antibody Testing Tests(Colloidal gold method,Enzyme linked immunosorbent assay and Chemiluminescent immunoassay).Methods From February 2020 to March 2020,39 outpatients and inpatients with2019 novel coronavirus diseases(COVID-19)in Kunming Third People′s Hospital were included as observation group.Fasting venous blood of all the enrolled subjects at different onset times was collected,and the samples were tested by three methods respectively,and the test results were statistically analyzed.Results The sensitivity of Colloidal gold method,Enzyme linked immunosorbent assay and Chemiluminescent immunoassay are 85.71%,86.81%and 81.32%respectively;the specificity of those three tests is 99.21%,95.24%and 97.22%respectively;and the area under(AUC)receiver operating characteristic curve(ROC curve)of Colloidal gold method,Enzyme linked immunosorbent assay and Chemiluminescent immunoassay is 0.925,0.938 and 0.965,respectively.These three methods have no statistical significance on the detection rate of 2019-nCoV antibody at different times of COVID onset.With the prolongation of the onset time,the detection rate of 2019-n CoV antibody by the three methods showed a trend of increasing first and then decreasing;Specifically,the detection rate reached a peak on the 15 th to 21 st day.Conclusion The accuracy of Chemiluminescent immunoassay is the highest in the diagnosis of COVID-19,and the diagnostic performance of these three methods can meet the clinical needs.
