摘 要:目的了解四川省棘球蚴病人群患病的变化情况。方法分别于2012年(以下简称"第1次")采用分层抽样调查和2015—2018年(以下简称"第2次")采用普查方法调查四川省35个棘球蚴病流行县人群患病情况。结果第1次调查112605人,确诊患者1311例,患病率为1.16%;第2次调查2744413人,确诊患者11924例,患病率为0.43%,两次调查均以甘孜州患病率最高,分别为1.59%(925/58121)和0.95%(10754/2744413);两次调查人群男女构成及年龄分布,存在一定差异,差异均有统计学意义(性别构成比:χ^(2)=203.338,P<0.001;年龄分布:χ^(2)=14619.868,P<0.001);第1次调查人群患病主要以CE为主,CE、AE标化患病率分别为0.84%和0.26%,第2次调查人群患病以AE为主,CE、AE患病率分别为0.21%(5737/2744413)和0.23%(6262/2744413),不同病灶类型棘球蚴病患病率均明显下降(CE:χ^(2)=10361.32,P<0.001;AE:χ^(2)=59.142,P<0.001);第1次调查男性和女性标化患病率分别为0.47%和0.65%,第2次调查男性和女性患病率分别为0.36%(4882/1353375)和0.5%(7012/1391038);不同性别棘球蚴病患病率均明显下降(男性:χ^(2)=3591.11,P<0.001;女性:χ^(2)=4711.20,P<0.001);两次调查均以0~19岁组患病率最低,分别为0.89%和0.15%,60~79组最高,分别为1.95%和0.83%,各年龄组人群棘球蚴病患病率均明显下降。结论由于诊断水平的提高和防治项目的强力实施,四川省棘球蚴病患者得到有效的诊治,人群患病情况有了较大的改善。Objects To analyze the changes in prevalence of echinococcosis in Sichuan Province in2012 and 2015 to 2018.Methods A stratified sampling method was used to investigate the prevalence of echinococcosis in 35 hydatid-endemic counties in Sichuan Province in 2012(the first time)and 2015 to 2018(the second time).Results In 2012 survey,112605 people were screened for echinococcusis infection by using abdominal B-type altrasonograph,and 1311 confirmed patients were found with a prevalence rate of 1.16%.In 2015 to 2018 survey,a total of 2744413 people were screened,and 11924 confirmed patients were diagnosed,with a prevalence rate of 0.43%.In the two surveys,the prevalence rate was the highest in Garze Tibetan autonomous prefecture,with a prevalence rate of 1.59%(925/58121)in 2012 survey and 0.95%(10754/2744413)in 2015 to2018 survey.There were significant differences in sex composition and age distribution in two surveys(sex ratio:χ^(2)=203.338,P<0.001;age distribution:χ^(2)=14619.868,P<0.001).In the first survey,the proportion of cystic echinococcosis(CE)was in dominant.The standardized prevalence rates of CE and alveolar echinococcosis(AE)in human were 0.84%and 0.26%,respectively.In the second survey,the number of AE cases was preponderating.The prevalence rates of CE and AE were 0.21%(5737/2744413)and 0.23%(6262/2744413),respectively.The prevalence of echinococcosis of different types decreased significantly(CE:χ^(2)=10361.32,P<0.001;AE:χ^(2)=59.142,P<0.001).The standardized prevalence rates of echinococcosis in males and females were 0.47%and 0.65%,respectively.In the second survey,the prevalence rates of echinococcosis in males and females were 0.36%(4882/1353375)and 0.5%,respectively(7012/1391038).The prevalence rates of echinococcosis in different genders decreased significantly(males:χ^(2)=3591.11,P<0.001;females:χ^(2)=4711.20,P<0.001).In the two surveys,the lowest prevalence was in age group less than 20 years,with prevalence rates of 0.88%in first survey and 1.92%in second survey.The highest prevalence wa
