摘  要:目的分析慢性心力衰竭患者血管生长因子的表达,明确氨基末端脑钠肽前体(NT-proBNP和心力衰竭的临床相关性。方法收集2019年6至12月首都医科大学附属北京友谊医院心内科住院的心力衰竭患者20例期入院的心功能,正常对照20例,抗体微阵列蛋白芯片检测血清中20种血管生长因子变化,将差异表达因子进行基因本体(GO)和KEGG富集通路分析;同时检测临床广泛认可的心力衰竭标志物NT-proBNP和炎症标志物超敏C反应蛋白(hsCRP)的表达,分析NT-proBNP、hsCRP水平与心脏结构和功能的关系。结果与对照组比较,慢性心力衰竭患者白细胞介素8(IL-8)、白细胞介素6(IL-6)、瘦素(LEPTIN),人上皮中性粒细胞活化肽78(ENA-78)、单核细胞趋化蛋白1(MCP-1)、肿瘤生长因子(GRO)和胰岛素样生长因子-1(IGF-1)水平升高,血管生成素(Ang)、碱性成纤维细胞生长因子(b FGF)、血小板生成素(TPO)、血管内皮生长因子(VEGF)和VEGF-D水平下降,进一步GO富集显示,细胞趋化因子和趋化因子调节差异表达明显,KEGG富集显示,细胞因子和细胞因子受体相互作用是差异表达最显著的通路。另外,心力衰竭患者NT-proBNP、hsCRP水平明显升高(P<0.001),其中NT-proBNP浓度与左心室舒张末期内径(LVEDD)呈正相关,与射血分数(EF)呈负相关(P均<0.01)。结论心力衰竭患者血管成长因子呈现差异表达,涉及多种炎症因子和细胞调节相关的受体通路。NT-proBNP与心脏重构损害程度相关。Objective To identify the expressed angiogenesis factors of patients with heart failure and probe into the correlation between NT-proBNP and heart failure. Methods From June 2019 to December 2019,20 patients with heart failure in the Department of Cardiology, Beijing Friendship Hospital, Capital Medical University and 20 normal controls were collected. Antibody microarray protein chip were used to detect the angiogenesis factors. The differentially expressed factors were analyzed for gene ontology(GO) and KEGG enrichment pathways. Meanwhile, the expression of the widely recognized heart failure marker NT-proBNP and the inflammation marker hsCRP were detected;the relationship between the levels of NT-proBNP, hsCRP and the patient’s cardiac structure and function were studied. Results The levels of serum IL-8, IL-6, LEPTIN,ENA-78, MCP-1, GRO, IGF-1 in the heart failure cases increased, and the levels of Ang, b FGF, TPO, VEGF,and VEGF-D decreased. Cell chemokine and chemokine regulation were identified by GO pathway;Cytokinecytokine receptor interaction was identified by KEGG pathway. In addition, the levels of NT-proBNP and hsCRP in patients with heart failure significantly increased(P<0.001), and the concentration of NT-proBNP was positively correlated with the left ventricular end-diastolic diameter(LVEDD) and negatively correlated with EF(P<0.01). Conclusions Various angiogenesis factors showe differential expression, involving a variety of inflammatory factors and cell regulation-related receptor pathways. NT-proBNP is closely related with the cardiac remodel.
