摘　　要：目的探讨动脉球囊扩张血管后内植支架治疗老年下肢闭塞性动脉硬化的效果。方法纳入2016年1月至2020年1月淮北市人民医院收治的下肢闭塞性动脉硬化患者50例,按照随机数字表法将患者分为观察组和对照组,各25例。比较两组患者临床疗效和并发症发生率;比较两组患者血流动力学指标、踝肱指数(ABI)、糖脂代谢指标。结果两组患者临床有效率和并发症发生率比较,差异无统计学意义(P>0.05)。观察组患者血细胞比容、红细胞沉降率、全血高切黏度和血浆黏度低于对照组患者,ABI高于对照组患者;观察组患者空腹血糖、餐后2 h血糖、糖化血红蛋白、甘油三酯、总胆固醇、低密度脂蛋白胆固醇低于对照组患者,高密度脂蛋白胆固醇高于对照组患者,差异有统计学意义(P<0.05)。结论动脉球囊扩张血管后内植支架治疗老年下肢闭塞性动脉硬化的效果显著,可有效改善患者血流动力学和糖脂代谢水平,且不额外增加并发症。Objective To investigate the effect of stent implantation after arterial balloon dilation in the treatment of lower extremity occlusive arteriosclerosis in elderly patients.Method A total of 50 patients with lower extremity arteriosclerosis obliterans admitted to Huaibei People's Hospital from January 2016 to January 2020 were selected.They were divided into observation group and control group according to random number table method,with 25 cases in each group.The clinical efficacy and complication rate of the two groups were compared.Hemodynamic indexes,ankle brachial index(ABI),glucose and lipid metabolism were compared between the two groups.Result There was no significant difference in clinical effective rate and complication rate between the two groups(P>0.05).Hematocrit,erythrocyte sedimentation rate,whole blood high-shear viscosity and plasma viscosity in the observation group were lower than those in the control group,and ABI was higher than that in the control group.Fasting blood glucose,2 h postprandial blood glucose,hemoglobin Alc,triglyceride,total cholesterol and low density lipoprotein cholesterol in the observation group were lower than those in the control group,and high density lipoprotein cholesterol was higher than those in the control group,with statistical significance(P<0.05).Conclusion Stent implantation after arterial balloon dilation is effective in the treatment of lower extremity occlusive arteriosclerosis in elderly patients,which can effectively improve the hemodynamics and glucose and lipid metabolism level of patients without additional complications.
