摘　　要：目的探讨基于前馈控制的全程无缝管理模式在老年糖尿病患者低血糖管理中的应用效果。方法采用便利抽样法,选取2019年8月—10月本科室收治的362例老年2型糖尿病患者中发生低血糖的120例患者为干预前基线资料,确定低血糖发生的高危环节,逐步建立并实施基于前馈控制的全程无缝管理模式。同时收集干预后2020年8月—10月本科室收治的老年2型糖尿病患者487例中发生低血糖的91例患者为干预后的资料。比较干预前后患者低血糖发生率及患者出院前自我管理行为得分的差异。结果干预后老年2型糖尿病患者低血糖发生率为18.7%(91/487)低于干预前的33.1%(120/362),干预前后比较,差异有统计学意义(P<0.05)。干预后患者出院时自我管理行为能力得分为(38.1±5.8)分高于干预前的(13.1±9.9)分,差异有统计学意义(P<0.05)。结论基于前馈控制的全程无缝管理模式应用于老年糖尿病患者低血糖管理中,有效降低了老年糖尿病患者低血糖发生率,提高了老年患者的自我管理能力,从而保障患者安全。Objective To explore the application effect of seamless management mode based on feedforward control in the management of hypoglycemia in elderly diabetic patients.Methods Using convenient sampling method,data of 120 patients with hypoglycemia in the 362 elderly type 2 diabetes patients from August to October 2019 were collected as the baseline.After analyzing high-risk factors of hypoglycemia,we gradually established and implemented the seamless management mode based on feedforward control.In order to compare the effects before and after the intervention,data of 91 patients with hypoglycemia out of the 487 elderly patients with type 2 diabetes admitted from August 2020 to October 2020 after the intervention were collected as post-intervention data.The incidence of hypoglycemia and score of diabetes self-care activities were compared before and after intervention.Results The incidence of hypoglycemia after intervention was lower than that before intervention(18.7%vs 33.1%),and the difference was statistically significant(P<0.01).After the implementation of the seamless management mode,the score of diabetes self-care activities was higher than the pre-intervention score[(38.1±5.8)vs(13.1±9.9)](P<0.05).Conclusions The seamless management mode based on feedforward control can effectively reduce the incidence of hypoglycemia in elderly patients with diabetes,improve the ability of self-care activities of diabetic elderly patients,so as to ensure patients safety.
