摘　　要：目的探讨hsa_circ_0008934在三阴性乳腺癌(TNBC)中的表达及其与新辅助化疗(NACT)治疗反应和预后的相关性。方法选取2016年2月至2019年2月河北省唐山市人民医院肿瘤医院收治的126例行NACT的TNBC患者作为研究对象。分析TNBC组织和癌旁组织中hsa_circ_0008934的表达情况。根据hsa_circ_0008934在TNBC中的表达情况将患者分为高表达组(n=73)和低表达组(n=53),比较两组的基线资料。将hsa_circ_0008934变量纳入和剔除分别建立NACT后TNBC未完全缓解的预测模型,并利用受试者工作特征(ROC)曲线对模型进行评价,利用Bootstrap重复取样方法对模型进行内部验证。结果TNBC组织中hsa_circ_0008934的表达水平明显高于癌旁组织,差异具有统计学意义(P<0.01);临床分期Ⅱ+Ⅲ期、有脉管瘤栓、肿瘤直径≥5cm、区域淋巴结转移、hsa_circ_0008934高表达、MP分级1~3级是NACT后TNBC未完全缓解的独立危险因素(P<0.05);纳入hsa_circ_0008934表达变量的模型具有更高的预测价值(P<0.05)。结论hsa_circ_0008934高表达的TNBC患者对NACT治疗反应不佳,且预后较差。hsa_circ_0008934表达水平可能提示TNBC患者NACT的反应和预后。Objective To explore the expression of hsa_circ_0008934 in triple negative breast cancer(TNBC)and its correlation with neoadjuvant chemotherapy(NACT)treatment response and prognosis.Methods A total of 126 patients with TNBC who underwent NACT in Tumor Hospital,Tangshan People's Hospital from February2016 to February 2019 were selected.The expression of hsa_circ_0008934 in cancer tissues and paracancerous tissues was analyzed.According to the expression of hsa_circ_0008934 in TNBC,patients were divided into high expression group(n=73)and low expression group(n=53),and the general clinical data of the two groups werecompared.The prediction model of incomplete remission of TNBC after NACT was established by including and eliminating hsa_circ_0008934 variables,and the model was evaluated by subject working characteristic curve(ROC),and was verified by Bootstrap repeated sampling method.Results The expression level of hsa_circ_0008934 in TNBC tissues was higher than that in paracancerous tissues(P<0.01).Clinical stageⅡ+Ⅲ,vascular tumor thrombus,tumor diameter≥5cm,regional lymph node metastasis,high expression of hsa_circ_0008934 andMP grade 1-3 were independent risk factors for incomplete remission after NACT treatment(P<0.05).The modelwith hsa_circ_008934 expression variables had higher predictive value(P<0.05).Conclusions TNBC patientswith high expression of hsa_cinc_0008934 had poor response to NACT treatment and poor prognosis.The level ofhsa_circ_008934 expression may inicate the response and prognosis of NACT therapy in patients with TNBC.
