重组人白细胞介素-2黏膜局部注射联合伐昔洛韦治疗复发性生殖器疱疹的疗效及对其免疫功能的影响

摘　　要：目的探讨重组人白细胞介素-2(rhIL-2)黏膜局部注射联合伐昔洛韦治疗复发性生殖器疱疹(RGH)的临床应用效果及对其免疫功能的影响。方法选取2020年5月至2021年5月深圳市龙岗区第三人民医院收治的70例RGH患者作为研究对象。按照随机数字表法分为治疗组(n=35)和对照组(n=35),对照组采用伐昔洛韦单一治疗,治疗组在对照组治疗基础上联合rhIL-2黏膜局部注射治疗。比较两组的临床疗效、临床症状改善时间、治疗前后细胞免疫功能和体液免疫功能、血清炎症因子水平及不良反应发生情况。结果治疗组治疗有效率高于对照组,差异具有统计学意义(P<0.05)。治疗组疼痛减轻时间、止疱时间、结痂时间均短于对照组,差异具有统计学意义(P<0.05)。治疗后,两组CD4^(+)、CD4^(+)/CD8^(+)、IgM、IgG、IgA水平均升高,CD8^(+)水平均降低,且治疗后治疗组CD4^(+)、CD4^(+)/CD8^(+)、IgM、IgG、IgA水平高于对照组,CD8^(+)水平低于对照组,差异具有统计学意义(P<0.05)。治疗后,两组血清肿瘤坏死因子-α(TNF-α)水平均降低,干扰素-γ(IFN-γ)水平均升高,且治疗后治疗组血清TNF-α水平低于对照组,IFN-γ水平高于对照组,差异具有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论rhIL-2黏膜局部注射联合伐昔洛韦能有效缩短RGH患者临床症状改善时间,在改善细胞免疫、体液免疫功能及减轻炎性反应方面均具有良好效果,且不会明显增加不良反应的发生情况,安全性好。Objective To investigate the efficacy of recombinant human interleukin-2(rhIL-2)mucosal local injection combined with valacyclovir in the treatment of recurrent genital herpes(RGH)and its effect on immunefunction.MethodsSeventy patients with RGH admitted to the Shenzhen Longgang District Third People's Hospitalfrom May 2020 to May 2021 were selected and randomly divided into treatment group(n=35)and control group(n=35).The control group was treated with valacyclovir monotherapy,and the treatment group was treated with rhIL-2 mucosal local injection and valacyclovir.The clinical efficacy,time to improvement of clinical symptoms,cellular immune function and humoral immune function before and after treatment,serum inflammatory factors andadverse reaction were compared between the two groups.Results The total effective rate in treatment group washigher than that in control group(P<0.05).The pain relief time,stopping time and scab time in the treatmentgroup were shorter than those in the control group(P<0.05).After treatment,the levels of CD4^(+),CD4^(+)/CD8^(+),IgM,IgG and IgA in both groups increased,while the levels of CD8^(+) decreased,the levels of CD4^(+),CD4^(+)/CD8^(+),IgM,IgG and IgA in the treatment group were higher than those in the control group,while the levels of CD8^(+) were lower than those in the control group(P<0.05).The levels of tumor necrosis factor-α(TNF-α)andinterferon-γ(IFN-γ)in both groups were decreased,and the levels of TNF-α in the treatment group were lower thanthose in the control group,while the levels of IFN-γ were higher than those in the control group(P<0.05).Therewas no significant difference in the incidence of adverse reactions between the two groups(P>0.05).Conclusions Rh IL-2 mucosal local injection combined with valacyclovir can effectively shorten the improvementtime of clinical symptoms in RGH patients,and has good effects in improving cellular immunity and humoral immunity functions and alleviating inflammatory reactions,without significantly increasing the occurrence
