产前三维超声在胎儿颅内病变诊断中的应用

摘　　要：目的:研究产前三维超声检查应用的胎儿颅内病变诊断中的价值。方法:本文调查时间选择2019年1月~2020年7月,将在此期间到本院进行产前超声检查的2000例孕周在20~24周的孕妇作为本文的研究对象,为所有研究对象进行产前三维超声检查胎儿的颅部,分析产前三维超声检查在胎儿颅内病变诊断中的价值。结果:(1)本文所有孕妇均随访到分娩和引产后,2000例研究对象当中存在胎儿颅内病变的病例53例,占2.65%。经过产前三维超声检查诊断出49例,漏诊4例,诊断率为92.45%。(2)产前三维超声诊断胎儿颅内病变的阳性预测值为97.96%、阴性预测值为99.74%、敏感度为90.57%、特异度为99.95%、准确度为99.7%、约登指数为90.51%、误诊率为0.05%、漏诊率为9.43%。结论:临床在进行产前胎儿颅内病变诊断的时候,通过产前三维超声进行诊断具有较高的诊断价值,能够直观、方便、安全快捷的了解到胎儿颅内的异常状况,是一种优质的影像学检查手段。Objective:To study the value of prenatal three-dimensional ultrasound in the diagnosis of fetal intracranial lesions.Methods:The survey time of this article is selected from January 2019 to July 2020.During this period,2000 pregnant women with gestational weeks between 20 and 24 weeks who came to our hospital for prenatal ultrasound examination were selected as the research objects of this article.Prenatal three-dimensional ultrasound examination of the fetus’s skull,analysis of the value of prenatal three-dimensional ultrasound in the diagnosis of fetal intracranial lesions.Results:(1)All pregnant women in this article were followed up after delivery and induction of labor.Among the 2000 study subjects,53 cases had fetal intracranial lesions,accounting for 2.65%.After prenatal three-dimensional ultrasound examination,49 cases were diagnosed and 4 cases were missed.The diagnosis rate was 92.45%.(2)Prenatal three-dimensional ultrasound diagnosis of intracranial lesions in the fetus has a positive predictive value of 97.96%,a negative predictive value of 99.74%,a sensitivity of 90.57%,a specificity of 99.95%,an accuracy of 99.7%,and a Youden index of 90.51%,the misdiagnosis rate was 0.05%,and the missed diagnosis rate was 9.43%.Conclusion:In the clinical diagnosis of prenatal fetal intracranial lesions,prenatal three-dimensional ultrasound diagnosis has high diagnostic value.It can intuitively,conveniently,safely and quickly understand the abnormal conditions of the fetus.It is a high-quality imaging methods.
