摘　　要：目的:分析18例卵巢纤维瘤的MRI表现及误诊原因,提高对卵巢纤维瘤的诊断水平。方法:回顾性分析经手术病理证实的卵巢纤维瘤的MRI表现,按瘤征分成五型并对误诊原因分析。结果:MRI表现:18例患者共19个肿瘤,①均质型:4例,实质肿块,长T1短T2信号,早期不强化,延迟期出现轻中度强化;②变性型:5例,囊实性混杂信号,含不规则长T1长T2信号区,瘤体的囊及实质部分强化不明显;③出血型:2例,T1W可见略高信号;④“Meigs综合征”型:5例,合并腹/胸腔积液,为实、囊实及囊性肿块,增强后不均匀延迟强化;⑤复杂型:2例,其中1例为双侧卵巢肿块。误诊共10例,误诊为浆膜下及阔韧带肌瘤、卵泡膜纤维瘤及颗粒细胞瘤、卵巢癌及浆液性囊腺瘤,分析原因为观察征象不仔细及对该病认识程度不够深刻。结论:不同类型的卵巢纤维瘤MRI有一定特征性,综合应用对肿瘤的定性诊断有重要意义。Objective:To improve the diagnostic level of ovarian fibroma by analyzing MRI findings and misdiagnosis reasons of 18 cases in our hospital.Methods:The MRI images of ovarian fibroma confirmed by operation and pathology were retrospectively analyzed.According to the tumor signs,they were divided into five types,and the causes of misdiagnosis were analyzed.Results:MRI findings:there were 19 tumors in 18 patients.①Homogeneous type:4 cases,solid mass,long T1 and short T2 signal,no enhancement in early stage,mild to moderate enhancement in delayed stage;②Degenerative type:5 cases,cystic and solid mixed signal,including irregular long T1 and long T2 signal area,the cystic and parenchymal part of the tumor were not significantly enhanced;③Hemorrhagic type:2 cases,T1W showed slightly high signal;④Meigs syndrome:5 cases were complicated with abdominal/pleural effusion,which were solid,cystic and solid masses with heterogeneous delayed enhancement after enhancement;⑤Complex type:2 cases,including 1 case of bilateral ovarian mass.10 cases were misdiagnosed as subserosal and broad ligament leiomyoma,follicular fibroma and granulosa cell tumor,ovarian cancer and serous cystadenoma.The reasons were that the signs were not carefully observed and the understanding of the disease was not deep enough.Conclusion:Different types of ovarian fibroma have certain characteristics on MRI,and comprehensive application is of great significance for the qualitative diagnosis of ovarian fibroma.
