摘 要:目的:探讨脑电双频谱指数(BIS)监测下经喉罩吸入七氟醚在小儿短小手术中的应用效果。方法:选择2018年2月~2020年1月在本院择期实行短小手术患儿32例作为研究对象,术前均未给予镇静药物。采用8%七氟醚+4L/min氧行麻醉诱导,待BIS显示下降至45~50时插入喉罩,术中吸入1.5%~2.5%七氟醚+2L/min氧,维持BIS在40~60。手术结束后,停止吸入七氟醚,待BIS恢复至70左右,拔除喉罩。记录麻醉前(T0)、诱导后(T1)、插喉罩即刻(T2)、手术切皮(T3)、术中(T4)、手术结束(T5)的心率(HR)、呼吸频率(RR)、脉搏氧饱和度(SpO_(2))指标的变化情况。结果:与T0相比,仅在T5时患儿HR、RR较快,组间差异明显,有统计学意义(P<0.05);患儿的苏醒时间为(14±6)min。术后均无恶心、呕吐、支气管痉挛以及术后躁动等现象发生。结论:在小儿短小手术中应用BIS监测经喉罩吸入七氟醚,具有起效迅速、苏醒快,对血流动力学影响较小的优势,可安全的应用于小儿短小手术中。Objective:To study the effect of sevoflurane inhalation through laryngeal mask under the monitoring of bispectral index(BIS)in children’s short-term surgery.Methods:From February 2018 to January 2020,32 children who underwent short-term surgery in our hospital were selected as the study objects,and no sedative was given before operation.Anesthesia was induced by 8%sevoflurane+4L/min oxygen.When BIS decreased to 45-50,laryngeal mask was inserted.During the operation,1.5%-2.5%sevoflurane+2L/min oxygen was inhaled to maintain bis at 40-60.After the operation,stop inhaling sevoflurane.When BIS recovers to about 70,remove the laryngeal mask.The changes of heart rate(HR),respiratory rate(RR),pulse oxygen saturation(SpO_(2))were recorded before anesthesia(T0),after induction(T1),immediately after inserting laryngeal mask(T2),during operation(T3),during operation(T4),and at the end of operation(T5).Results:Compared with T0,HR and RR were faster only in T5,the difference between groups was significant(P<0.05),and the recovery time of children was(14±6)min.There were no nausea,vomiting,bronchospasm and restlessness after operation.Conclusion:The application of BIS monitoring inhalation of sevoflurane through laryngeal mask in children’s short-term surgery has the advantages of rapid onset,quick recovery and little impact on hemodynamics.It can be safely applied in the short-term surgery of the second child,and it is worth popularizing in clinical practice.
