营养医师分区责任制管理成效分析

摘　　要：目的为加强住院患者营养支持的管理工作,2018年8月开始我院在临床科室设置营养医师,落实营养医师分区责任制管理,一方面加强对住院患者营养筛查、评估、营养干预及动态监测,根据患者病情提供个性化营养治疗方案,进一步促进患者营养支持序贯性治疗,从而降低营养不良发生率;另一方面加强营养科与全院各临床科室的专业交流,提高临床科室对营养科关键工作的知晓度,推动我院临床营养科工作的顺利开展。方法将营养医师分区责任制管理对策实施前后1年的关键指标进行对比,即2017年8月—2018年7月(营养医师分区责任制管理实施前一年)与2018年8月—2019年7月(营养医师分区责任制管理实施1年)的营养查房、营养会诊、肠内营养配置、营养科关键工作知晓率、平均住院日进行对比,了解营养医师分区责任制实施后的成效。结果医院通过营养医师分区责任制管理,明显提高了营养科会诊量、查房总人数及肠内配置量,进一步降低了患者营养不良发生率,缩短患者平均住院时间,增加了入院人数,提高了各临床医护人员对临床营养科关键工作的知晓率及对营养科工作的支持度。结论营养医师分区责任制管理是营养科精细化管理的重要举措之一,是为患者提供个性化且序贯治疗的必要条件,是提高营养科与临床各科室高效协作的重要举措,更是降低患者营养不良发生率的重要举措。Objective In order to strengthen the management of nutritional support for inpatients,Since August 2018,our hospital has set up nutritionists in clinical departments,In order to implement the responsibility system for nutrition physicians in different areas,On the one hand,we should strengthen the nutritional screening,evaluation and nutritional ward round of hospitalized patients,and provide individualized nutritional treatment program according to the patient's condition,So as to promote the sequential treatment of nutritional support and reduce the incidence of malnutrition;On the other hand,strengthen the professional communication between nutrition department and clinical department,improve the awareness of nutrition work,and promote the smooth development of nutrition work.Methods The key indicators of the nutrition physician in the first year before and after the implementation of the management strategy were compared,That is,from August 2017 to July 2018 and from August 2018 to July 2019,nutrition ward round,nutrition consultation,enteral nutrition allocation,awareness rate of key work in nutrition department,average length of stay and average cost were compared,Objective to understand the effect of the implementation of regional responsibility system for nutritionists.Results Through the division responsibility system management of nutritionists,the consultation volume,the total number of rounds and the amount of intestinal allocation in the Department of nutrition were significantly improved,and the incidence of malnutrition was reduced,Shorten the average length of stay of patients,Increased the number of admissions,and further improve the awareness rate of key work of clinical nutrition department and support for the work of nutrition department.Conclusion The management of the division responsibility system of nutritionists is one of the measures of fine management of nutrition department,is the necessary condition for individualized and sequential treatment for patients,an important measure to impro
