摘　　要：目的探讨64排螺旋CT诊断法洛四联症的价值。方法选取2012年5月至2016年5月江西省儿童医院收治的80例法洛四联症患儿作为研究对象,患儿均接受超声心动图及64排螺旋CT血管造影检测。以手术结果为金标准,比较超声心动图及64排螺旋CT血管造影的检测结果,分析二者的诊断价值。结果经过手术证实,患儿心血管畸形有331处。病变类型包括右心室增肥、室间隔缺损、肺动脉狭窄、主动脉骑跨。64排螺旋CT血管造影检测对肺动脉狭窄、室间隔缺损、主动脉骑跨、右心室增肥的诊断灵敏度、特异度及准确度均高于超声心动图,差异有统计学意义(P<0.05)。结论64排螺旋CT血管造影检查诊断法洛四联症患儿的准确度、特异度与灵敏度均较高,可为临床早期手术提供准确有效的依据,提高患儿生存率。Objective To explore the value of 64-slice spiral CT in the diagnosis of tetralogy of Fallot.Methods A total of 80 children with tetralogy of Fallot treated in Jiangxi Children′s Hospital from May 2012 to May 2016 were selected as the research subjects,all children received echocardiography and 64-slice spiral CT angiography.The results of echocardiography and 64-slice spiral CT angiography were compared using surgical results as the gold standard,the diagnostic value of the two methods was analyzed.Results After surgery,there were 331 cardiovascular malformations.The types of lesions included right ventricular hypertrophy,ventricular septal defect,pulmonary artery stenosis,and aortic straddle.The diagnostic sensitivity,specificity and accuracy of 64-slice spiral CT angiography were higher than those of echocardiography in pulmonary artery stenosis,ventricular septal defect,aortic riding,and right ventricular enlargement,the differences were statistically significant(P<0.05).Conclusion 64-slice spiral CT angiography has a high diagnostic accuracy rate for children with tetralogy of Fallot,and its specificity and sensitivity were high,which provides accurate and effective basis for early clinical surgery and improves the survival rate of children.
