李用粹《证治汇补》论治癃闭的学术特色

摘　　要：癃闭相当于现代医学的尿潴留,是临床常见的急症之一。虽然在现代医学中有很多侵入性的方法可以治疗癃闭,但是目前临床上也存在很多暂时性、非器质性改变导致的癃闭。中医药治疗具有鲜明的优势,李用粹所著《证治汇补》是其汇集前人经典的基础上总结自己多年行医的心得体会汇编而成,该书对包括癃闭在内的各种内科杂病的病因病机、诊断治法以及用药等方面均有详细的论述。本文主要分析李用粹关于癃闭的认识、诊断及治疗。李用粹认为治疗癃闭需要审证辨因、分证施治、内外兼治,本文在《证治汇补》原文的基础上结合前人的临床诊疗经验,从癃闭的内因、外候、脉诊、治癃三法、遣方用药等几个方面分别总结分析了李用粹治疗癃闭的中医学术思想,以期为癃闭的临床诊疗提供帮助。Uroschesis is equivalent to urinary retention in modern medicine and is one of the common clinical emergencies.Although there are many invasive methods in modern medicine that can treat uroschesis,but there are also many clinical temporary,non-organic changes caused by the uroschesis.Traditional Chinese medicine treatment has distinct advantages,The Zhengzhi Huibu by LI Yong-cui compiled from a summary of his experience in medical practice for many years on the basis of collecting previous classics,the book has a detailed discussion on the etiology,diagnosis,treatment,and medication of various medical and miscellaneous diseases,including uroschesis.This paper mainly analyzes LI Yong-cui′s understanding,diagnosis and treatment of uroschesis.LI Yong-cui believes that the treatment needs evidence identification,evidence treatment,both internal and external treatment,this paper based on the original text of Zhengzhi Huibu combined with the clinical experience of predecessors,the academic thought of LI Yong-cui was summarized and analyzed from the aspects of internal and external factors,external ology,pulse diagnosis,treatment,and prescription medication,in order to provide help for closed clinical diagnosis and treatment.
