摘  要:目的:观察利拉鲁肽对老年糖尿病患者血糖水平、血清炎症因子、氧化应激及肠道菌群的影响。方法:选取2019年12月至2020年4月南阳市第二人民医院收治的90例老年2型糖尿病患者为研究对象。采用随机数字表法分为试验组和对照组,每组45例。试验组给予利拉鲁肽治疗,对照组给予瑞舒伐他汀治疗。比较两组患者治疗前后血糖水平、血清炎症因子、氧化应激指标和肠道菌群的差异。结果:治疗后试验组患者空腹血糖(fasting blood glucose,FBG)、餐后2 h血糖(2-hour postprandial blood glucose,2h-PBG)、糖化血红蛋白(glycosylated hemoglobin,HbA1C)水平均显著低于对照组(P<0.05)。试验组患者肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)、C-反应蛋白(C-reactive protein,CRP)、白介素6(interleukin-6,IL-6)水平显著低于对照组(P<0.05),白介素10(interleukin-10,IL-10)水平显著高于对照组(P<0.05)。试验组患者血清丙二醛(malondialdehyde,MDA)和超氧化物歧化酶(superoxide dismutase,SOD)水平、总抗氧化能力(total antioxidant capacity,T-AOC)较对照组显著升高(P<0.05)。试验组患者粪便肠杆菌、肠球菌、酵母菌检出值显著低于对照组(P<0.05),双歧杆菌、拟杆菌、乳杆菌检出值显著高于对照组(P<0.05)。治疗期间,试验组患者药品不良反应发生率与对照组比较差异无统计学意义(P>0.05)。结论:老年糖尿病患者采用利拉鲁肽治疗,可有效控制血糖水平,改善炎症反应和氧化应激状态,调节肠道菌群,且安全性较高。Objective:To observe the effects of liraglutide on blood glucose level,serum inflammatory factors,oxidative stress and intestinal flora in elderly patients with diabetes mellitus.Methods:A total of 90 elderly patients with type 2 diabetes mellitus admitted to Nanyang Second People’s Hospital from December 2019 to April 2020 were selected as research subjects,and were randomly divided into the experimental group and the control group,each consisting of 45 patients.The patients in the experimental group and the control group were treated with liraglutide and rosuvastatin respectively.The differences in blood glucose levels,serum inflammatory factors,oxidative stress indexes and intestinal flora between the two groups were compared before and after treatment.Results:After treatment,the levels of fasting blood glucose(FBG),2-hour postprandial blood glucose(2h-PBG)and glycosylated hemoglobin(HbA1c)in the patients of the experimental group were significantly lower than those in the patients of the control group(P<0.05).The levels of tumor necrosis factor-α(TNF-α),C-reactive protein(CRP)and interleukin-6(IL-6)in the patients of the experimental group were significantly lower than those in the patients of the control group(P<0.05),and the level of interleukin-10(IL-10)in the patients of the experimental group was significantly higher than that of the latter(P<0.05).The levels of malondialdehyde(MDA),superoxide dismutase(SOD)and total antioxidant capacity(T-AOC)in the serum of the patients in the experimental group were obviously higher than those of the control group(P<0.05).The detection rates of Enterobacteria,Enterococcus and Yeast in feces of the patients in the experimental group were significantly lower than those of the control group(P<0.05),while the detection rates of Bifidobacteria,Bacteroidetes and Lactobacillus were significantly higher than those of the control group(P<0.05).During treatment,there was no statistical significance in the rate of adverse drug reactions,when comparisons were made between the
