摘　　要：目的探讨机采血小板输注无效的原因,为临床血小板的有效输注提供数据支持。方法选择2018年1月1日至2019年12月31日在本地区医院首次输注机采血小板的住院患者为研究对象,从疾病类型、年龄、临床因素及输注次数情况对患者血小板输注疗效进行分析。结果329例住院患者机采血小板输注无效123例,输注无效率为37.39%。不同疾病类型、年龄、临床因素及输注次数患者间输注无效率比较,差异有统计学意义(χ^(2)=24.456、15.963、19.517、47.131,P=0.000、0.001、0.000、0.000)。不同性别组间输注无效率比较,差异无统计学意义(χ^(2)=0.112,P=0.738)。结论原发病、感染、发热、多次输注等因素均可导致血小板输注无效,应针对不同影响因素积极治疗以提高血小板输注效果。Objective To explore the causes of machine-collected platelet transfusion refractoriness(PTR),so as to provide data support for the effective transfusion of platelets in clinic.Methods Inpatients who were transfused with machine-collected platelets for the first time in this regional hospital from January 1,2018 to December 31,2019 were selected as study subjects.The efficacy of platelet transfusion in patients was analyzed in terms of disease type,age,clinical factors and number of transfusions.Results There were 123 cases of machine-collected PTR in 329 inpatients,with the rate of ineffective transfusion of 37.39%.There were statistically significant differences in the rate of ineffective transfusion among patients with different disease types,age,clinical factors and number of transfusions(χ^(2)=24.456,15.963,19.517,47.131,respectively,P=0.000,0.001,0.000,0.000,respectively).In addition,there was no statistically significant difference in the rate of ineffective transfusion between the groups of different gender(χ^(2)=0.112,P=0.738).Conclusion Primary disease,infection,fever,and multiple transfusions can lead to PTR,and these influencing factors should be treated actively to improve the efficacy of platelet transfusion.
