耳穴电针联合角调式音乐治疗ICU谵妄的临床研究

摘　　要：目的观察耳穴电针联合角调式音乐干预ICU谵妄患者的有效性。方法将63例ICU谵妄患者随机分为干预组与对照组。干预组以耳穴电针刺激联合角调式音乐治疗,对照组予右美托咪定注射液持续泵入治疗,每日予ICU谵妄筛查量表(ICD-SC)、ICU意识模糊评估量表(CAM-ICU)及利兹睡眠评价量表(LSEQ)各评分1次,连续5 d,观察两组患者症状缓解情况。结果干预组改善ICU谵妄的5 d有效率高于对照组,但差异无统计学意义(P>0.05)。干预组谵妄持续时间明显小于对照组(P<0.05)。治疗5 d后干预组利兹睡眠评价量表(LSEQ)评分明显高于对照组(P<0.05)。两组患者的ICU住院时间及机械通气时间差异无统计学意义(P>0.05)。对照组2例患者出现心动过缓,干预组未发生明显不良反应。结论耳穴电针联合角调式音乐是干预ICU谵妄的有效方法,且无明显不良反应。Objective:To investigate the efficacy of ear electroacupuncture and Jiao mode Chinese music in patients with ICU delirium.Methods:63 patients with ICU delirium were randomly divided into two groups,the intervention group and control group.Patients in intervention group were treated with electric acupuncture at ear acupoints and Jiao mode Chinese music therapy.Patients in control group were treated with continuous dexmedetomidine injection.The Intensive Care Delirium Screening Checklist(ICD-SC),Confusion Assessment Method for the Intensive Care Unit(CAM-ICU),and Leeds Sleep Evaluation Questionnaire(LSEQ)were used to evaluate the therapeutic effect of two groups every day.Results:The 5-day effective rate of intervention group was higher than that of the control group,but with no significant difference(P>0.05).The duration of ICU delirium in the intervention group was significantly lower than that in the control group(P<0.05).The LSEQ score of the intervention group was significantly higher than that of the control group after 5 days of treatment(P<0.05).There was no significant difference in ICU stay time or mechanical ventilation time between two groups(P>0.05).Two patients in the control group had bradycardia.No obvious adverse reactions occurred in the intervention group.Conclusion:Electroacupuncture of ear acupoint combined with Jiao mode Chinese music is an effective method to treat ICU delirium without obvious adverse reaction.
