摘 要:目的探讨2型糖尿病(T2DM)患者血清尿酸(UA)水平与血糖波动的关系。方法收集2018年12月—2020年01月在苏州大学附属第一医院内分泌科住院的T2DM患者156例(年龄25~65岁;男83例,女73例),将患者根据血清UA水平三分位等分为3组,UA1组(UA<276.21μmol/L,n=52),UA2组(276.21μmol/L≤UA<366.70μmol/L,n=52),UA3组(UA≥366.70μmol/L,n=52),比较三组资料,分析UA水平与血糖波动指标(SD、MAGE和CV)的关系。结果三组间HbA1C、FCP、P2CP、TG、HDL-C、SD、CV、MAGE差异具有统计学意义(P<0.05)。UA1组的FCP、P2CP、TG水平低于UA3组,而HbA1C、HDL-C、SD、CV、MAGE水平高于UA3组。UA2组的FCP、P2CP水平低于UA3组(P<0.05)。在相关分析中,UA与SD、MAGE、CV呈负相关(P<0.05)。分别以SD、MAGE、CV为因变量,多重线性回归结果显示UA是SD、MAGE、CV的影响因素,随着UA的增加,SD、MAGE、CV逐渐降低。结论血清UA水平减少是T2DM患者血糖波动增加的独立危险因素,维持适量的UA水平可减轻血糖波动。Objective To investigate the relationship between serum uric acid(UA)and continuous glucosemonitoring parameters in patients with type 2 diabetes mellitus(T2DM).Methods A total of 156 T2DM patients ranging from 25 to 65 years old,hospitalized in the First Affiliated Hospital of Soochow University from December 2018 to January 2020 were enrolled in this study.After quantifying serum UA,83 male and 73 female T2DM patients were divided into group UA1(n=52,UA<276.21μmol/L),group UA2(n=52,276.21μmol/L≤UA<366.70μmol/L)and group UA3(n=52,UA≥366.70μmol/L).The clinical data were collected and compared among three groups,and the relationship between UA and continuous glucose-monitoring parameters were analyzed.Results There was statistical difference between the HbA1C,FCP,P2CP,TG,HDL-C,SD,CV and MAGE of the three groups(P<0.05).The FCP,P2CP and TG levels in UA1 group were lower than those in UA3 group,while the HbA1C,HDL-C,SD,CV and MAGE levels were higher than UA3 group.The FCP and P2CP levels of UA2 group were lower than those of UA3 group(P<0.05).In the correlation analysis,UA was negatively correlated with SD,MAGE and CV(P<0.05).Taking SD,MAGE and CV as dependent variables,the results of multiple linear regression showed that UA was the influencing factor of SD,MAGE and CV.With the increase of UA,the SD,MAGE and CV decreased gradually.Conclusion The decrease of serum UA level is an independent risk factor for the increase of blood glucose fluctuation in patients with T2DM.Maintaining a moderate level of UA can reduce blood glucose fluctuation.
