脓血症患者中炎症指标联合血栓弹力图的应用分析

摘　　要：目的探讨炎症指标联合血栓弹力图在脓血症患者的作用。方法选取2016年4月—2019年4月78例脓血症患者进行研究,检测所有患者炎症因子和凝血功能指标,予以血栓弹力图检查,根据凝血综合指数分为对照组(n=30)、低凝组(n=22)和高凝组(n=26),比较三组炎症因子、凝血功能和血栓弹力图指数。结果三组C反应蛋白、凝血酶原时间和纤维蛋白原水平差异无统计学意义(P>0.05),低凝组患者降钙素原、内毒素、血小板计数、血块生成率和血块强度值高于对照组和高凝组,凝血时间和血块生成时间低于对照组和高凝组(P<0.05);三组D-二聚体水平对比,对照组最低,高凝组最高,差异有统计学意义(P<0.05)。结论联合应用炎症指标和血栓弹力图可准确判断脓血症患者凝血功能,对疾病严重程度做出准确评估,以尽早处理,改善预后。Objective To investigate the effect of inflammatory indicators combined with thromboelastogram in patients with sepsis.Methods From April 2016 to April 2019,78 patients with sepsis were selected.All patients were tested for inflammatory factors and coagulation function indexes,and thromboelastogram was performed.According to the coagulation indexes,patients were divided into control group(n=30),low condensation group(n=22)and high condensation group(n=26),to compare inflammation factors,blood coagulation function and thrombus elastic figure index.Results There was no difference in C-reactive protein,prothrombin time and fibrinogen levels among the three groups(P>0.05).Calcitonin,endotoxin,platelet count,blood clot formation rate and blood clot intensity in low condensation group were higher than those in control group and high condensation group,and the clotting time and blood clot formation time were lower than those in control group and high condensation group(P<0.05).The D-Dimer level was the lowest in control group and the highest in high condensation group,and the difference was statistically significant(P<0.05).Conclusion The combined use of inflammatory markers and thromboelastogram can accurately determine the coagulation function of patients with sepsis,and accurately evaluate the severity of the disease,so as to treat as soon as possible and improve the prognosis.
