乙肝血清标志物、HBV-DNA对诊断乙型肝炎病毒的临床研究

摘　　要：目的探讨乙肝血清标志物、HBV-DNA、ALT对诊断乙型肝炎病毒的价值研究。方法选择2017年12月—2019年1月赣州市肿瘤医院收治的172例已确诊的乙肝患者,血清标志物采用化学发光法测定,包含:HBsAg、HBsAb、HBeAg、HBeAb、HBcAg、HBcAb,同时运用荧光定量PCR仪检测HBV-DNA定量,采用IFCC双试剂酶法检测ALT。结果RT-PCR检测HBV-DNA发现,血清标志物模式6~12中未出现HBV-DNA阳性患者15例,模式1中HBV-DNA阳性患者48例占比75.0%,模式4中HBV-DNA阳性患者47例占比66.2%,模式2中HBV-DNA阳性患者1例占比50.0%,模式5仅1例,模式1、2中HBV-DNA含量较模式3、4、5有明显增多。结论HBV-DNA载量、血清标志物、ALT三种检测指标有密切联系,分别反映疾病的不同类型,检测血清标志物、HBV-DNA载量和ALT水平,在HBV感染医治中具有重要意义。Objective To explore the value of hepatitis B serum markers,HBV-DNA,ALT in the diagnosis of hepatitis B virus.Methods 172 patients with hepatitis B diagnosed in Ganzhou Cancer Hospital were selected from December 2017 to January 2019.Serum markers were determined by chemiluminescence method,which included:HBsAg,HBsAb,HBeAg,HBeAb,HBcAg and HBcAb were detected simultaneously in 172 patients.HBV-DNA was quantitatively detected by fluorescence quantitative PCR,and ALT was detected by IFCC double reagent enzyme method.Results RT-PCR detection of HBV-DNA showed that there were 15 HBV-DNA positive patients in serum marker pattern 6-12,48 HBV-DNA positive patients in pattern 1(75.0%),47 HBV-DNA positive patients in pattern 4(66.2%).The proportion of HBV-DNA positive patients in mode 2 was 1(50.0%),and the proportion of HBV-DNA positive patients in mode 5 was only 1.The content of HBV-DNA in mode 1 and 2 was significantly higher than that in mode 3,4 and 5.Conclusion HBV-DNA load,serum markers and ALT are closely related to each other,reflecting different types of disease respectively.Detection of serum markers,HBV-DNA load and ALT level is of great significance in the treatment of HBV infection.
