摘  要:目的探讨Karl迭代重建联合低电压和自动管电流调节(ATCM)技术用于外伤性肋骨骨折的可行性。方法50例因胸部外伤行CT检查患者,按随机数字表分为A、B两组,每组各25例。A组为常规组,采用120 kV、固定管电流140 mAs和滤波反投影(FBP)重建;B组为低剂量组,采用100 kV、uDose智能毫安调节技术和Karl迭代重建,其他扫描参数2组一致。比较2组间CT值、客观图像噪声、信噪比(SNR)、对比噪声比(CNR)、图像诊断接受率、主观图像噪声评分、骨折分度、扫描长度、平均管电流、CT容积剂量指数(CTDIvol)、剂量长度乘积(DLP)和有效量(ED)。结果2组患者一般资料具有可比性。B组平均管电流、CTDIvol、DLP和ED均低于A组(均P<0.01),其中ED为(1.65±0.62)mSv,较A组(5.67±1.65)mSv降低70.9%。2组图像CT值、客观图像噪声、SNR、CNR、图像诊断接受率、主观图像噪声评分、骨折分度和扫描长度差异均无统计学意义(均P>0.05)。结论Karl迭代重建联合100 kV和ATCM技术在保证外伤性肋骨骨折诊断图像质量的前提下,显著降低了辐射剂量。Objective To investigate the feasibility of Karl iterative reconstruction combined with lowvoltage and automatic tube current modulation technique(ATCM)in rib fractures.Methods 50 patients presented with thoracic trauma were randomly divided into group A(conventional group,25 cases)and group B(low-dose group,25 cases).Group A was scanned according to the protocol of 120 kV,140 mAs and filtered back projection(FBP)reconstruction,and group B was scanned with 100 kV,uDose ATCM and Karl iterative reconstruction,while the other parameters kept consistent.CT value,objective image noise,signal-to-noise ratio(SNR),contrast to-noise ratio(CNR),image diagnostic acceptability,subjective image noise score,fracture scale,scan length,mean tube current,volume CT dose index(CTDIvol),dose length product(DLP)and effective dose(ED)were compared between the two groups.Results No significant difference was found in general information between the two groups.The mean tube current,CTDIvol,DLP and ED of group B were lower than those of group A(all P<0.01),and the ED reduction of 70.9%was obtained in group B(1.65±0.62)mSv compared with group A(5.67±1.65)mSv.There were no significances in CT value,objective image noise,SNR,CNR,image diagnostic acceptability,subjective image noise score,fracture scale and scan length between the two groups(all P>0.05).Conclusion Karl iterative reconstruction combined with 100 kV and ATCM technique can significantly reduce the radiation dose while maintain the image quality required for diagnosis of traumatic rib fractures.
