改良经跗骨窦微创小切口切开复位钢板内固定术治疗跟骨骨折的效果研究

摘　　要：目的:研究用改良经跗骨窦微创小切口切开复位钢板内固定术治疗跟骨骨折的效果。方法:选择昆山市中西医结合医院收治的40例跟骨骨折患者作为观察对象。按照术中所做手术切口的不同将其分为常规组和改良组。用改良经跗骨窦微创小切口切开复位钢板内固定术对改良组患者进行治疗,用常规的L形外侧切口切开复位钢板内固定术对常规组患者进行治疗,然后比较两组患者的各项临床指标。结果:改良组患者术中的失血量少于常规组患者,其手术的时间和术后骨折愈合的时间均短于常规组患者,其术后并发症的发生率低于常规组患者,P<0.05。术后1个月,改良组患者美国足踝骨科协会(AOFAS)踝与后足功能评分系统的评分高于常规组患者,其视觉模拟评分法(VAS)的评分低于常规组患者,P<0.05。结论:用改良经跗骨窦微创小切口切开复位钢板内固定术治疗跟骨骨折的效果显著,能减轻患者术后的疼痛感,促进其踝关节功能的恢复,且具有术中失血量少、手术时间和术后骨折愈合时间短及患者术后的并发症少等优点。Objective:To study the effect of modified micro-incision through tarsal sinus open reduction and plate internal fixation for calcaneal fracture.Methods:40 cases of calcaneal fracture in Kunshan Integrated Traditional Chinese and Western Medicine Hospital were selected as the observation object.The patients were divided into conventional group and modified group according to the incision.Patients in the modified group were treated with modified transtarsal sinus minimally invasive incision open reduction and plate internal fixation,and patients in the conventional group were treated with conventional L-shaped lateral incision open reduction and plate internal fixation,and then the clinical indicators of the two groups were compared.Results:The blood loss in the modified group was less than that in the conventional group,the operation time and postoperative fracture healing time were shorter than that in the conventional group,and the incidence of postoperative complications was lower than that in the conventional group,P<0.05.One month after surgery,the scores of the American Foot and Ankle Orthopaedic Association(AOFAS)ankle and hindfoot function scoring system in the modified group were higher than those in the conventional group,and the visual analog scale(VAS)scores were lower than those in the conventional group,P<0.05.Conclusion:The treatment of calcaneal fracture with modified trans-tarsal sinus minimally invasive incision open reduction and plate internal fixation has a significant effect,can reduce the postoperative pain of patients,promote the recovery of ankle joint function,and has the advantages of less intraoperative blood loss,short operation time and postoperative fracture healing time,and fewer postoperative complications.
