超声彩色室壁运动技术联合血浆脑钠肽检测评估高血压患者左心室舒张功能的效果

摘　　要：目的:研究用超声彩色室壁运动技术(CK)联合血浆脑钠肽(BNP)检测评估高血压患者左心室舒张功能的效果。方法:将80例高血压患者作为研究对象。对其均进行超声心动图检查,根据其肺静脉及二尖瓣舒张期多普勒血流频谱将其分为舒张功能正常组(A组)、舒张功能受损组(B组)和舒张功能假性正常组(C组)。进行超声心动图检查时,采用超声CK测量舒张期前1/3各节段心内膜位移面积的百分比(1/3RFAC)及其偏差(1/3RFAC-sd),以评价三组患者的左心室舒张功能,同时检测并比较三组患者血浆BNP的水平。结果:B组患者与C组患者的1/3RFAC均低于A组患者,其1/3RFAC-sd均高于A组患者,P<0.05。C组患者的1/3RFAC低于B组患者,其1/3RFAC-sd高于B组患者,P<0.05。C组患者血浆BNP的水平高于B组患者,P<0.05。B组患者血浆BNP的水平高于A组患者,P<0.05。结论:采用超声CK联合血浆BNP检测能有效地评估高血压患者左心室的舒张功能。Objective:To study the effect of color echocardiography(CK)combined with plasma brain natriuretic peptide(BNP)in evaluating left ventricular diastolic function in patients with hypertension.Methods:80 patients with hypertension were studied.All of them were examined by echocardiography and divided into normal diastolic group(group A),impaired diastolic group(group B)and pseudonormal diastolic group(group C)according to pulmonary vein and mitral valve diastolic Doppler flow spectrum.During echocardiography,ultrasound CK was used to measure the percentage of endocardial displacement area(1/3RFAC)and deviation(1/3 RFA-SD)of each segment in the first 1/3 of diastolic period to evaluate the left ventricular diastolic function of patients in the three groups,and to dete ct and compare the plasma BNP levels of patients in the three g roups.Results:RFAC of group B and GROUP C was lower than that of group A,and 1/3 RFA-SD was higher than that of group A,P<0.05.1/3RFAC in group C was lower than that in group B,and 1/3 RFA-SD was higher than that in group B,P<0.05.The plasma BNP level in group C was higher than that in group B,P<0.05.The plasma BNP level in group B was higher than that in group A,P<0.05.Conclusion:Ultrasound CK combined with plasma BNP can effectively evaluate left ventricular diastolic function in hypertensive patients.
