糖肾康膏方治疗禁用二甲双胍片的2型糖尿病肾病患者的疗效

摘　　要：目的观察糖肾康膏方治疗禁用二甲双胍片的2型糖尿病肾病患者的疗效。方法将80例禁用二甲双胍片的2型糖尿病肾病患者随机分为对照组和观察组,每组40例。对照组给予门冬胰岛素注射液和甘精胰岛素注射液皮下注射治疗,观察组在对照组基础上,加用糖肾康膏方口服。两组疗程均为3个月。比较两组患者治疗前后空腹血糖、餐后2 h血糖、糖化血红蛋白(HbA1c)、尿蛋白与肌酐比值(ACR)、预估肾小球滤过率(eGFR)、空腹胰岛素(FINS)、稳态模型评估胰岛素抵抗(HOMA-IR)指数、稳态模型胰岛β细胞功能(HOMA-β)及体质指数,记录两组患者治疗期间的每日胰岛素剂量及低血糖发生的情况。结果治疗后,两组患者eGFR、FINS、HOMA-β水平高于治疗前,且观察组eGFR、HOMA-β水平高于对照组,两组患者空腹血糖、餐后2 h血糖、HbA1c、ACR水平低于治疗前,且观察组上述指标低于对照组(均P<0.05),对照组HOMA-IR指数及体质指数高于治疗前,而观察组HOMA-IR指数及体质指数低于治疗前及对照组(均P<0.05)。观察组每日胰岛素剂量及低血糖发生率低于对照组(P<0.05)。结论在胰岛素治疗的基础上,给予糖肾康膏方治疗禁用二甲双胍片的2型糖尿病肾病患者可以有效地控制血糖、减轻胰岛素抵抗、改善胰岛β细胞功能、降低低血糖风险、改善肾功能。Objective To observe the efficacy of Tangshenkang Herbal Paste for the treatment of type 2 diabetic nephropathy patients with a ban on metformin tablets use.Methods Eighty patients suffering from type 2 diabetic nephropathy with a ban on metformin tablets use were randomly divided into control group and observation group,with 40 cases in each group.The control group was subcutaneously injected with Insulin Aspart Injection and Insulin Glargine Injection,based on which the observation group was orally administrated Tangshenkang Herbal Paste additionally.The treatment lasted for three months in both groups.The two groups were compared in terms of FBG,2-hour postprandial blood glucose,hemoglobin A1c(HbA1c),albuminuria-to-creatinine ratio(ACR),estimated glomerular filtration rate(eGFR),fasting insulin(FINS),homeostasis model assessment of insulin resistance(HOMA-IR)index,homeostasis model assessment isletβcell function(HOMA-β),and body mass index before and after the treatment.The daily insulin dose and the incidence of hypoglycemia during the treatment were recorded in the two groups.Results After the treatment,the levels of eGFR,FINS,and HOMA-βin both groups were higher than those before the treatment,and the observation group yielded increased eGFR and HOMA-βlevels than the control group;the levels of FBG,2-hour postprandial blood glucose,HbA1c,and ACR were lower in the two groups as compared with those before the treatment,and the observation group exhibited lower aforementioned indices than the control group(all P<0.05);HOMA-IR index and body mass index in the control group were higher than those before the treatment,whereas the observation group had lower HOMA-IR index and body mass index than the pre-treatment and the control group(all P<0.05).The daily insulin dose and the incidence rate of hypoglycemia in the observation group were lower than those in the control group(P<0.05).Conclusion On the basis of insulin therapy,Tangshenkang Herbal Paste can effectively control blood glucose,reduce insulin resista
